Often in our office when dealing with an immediate implant placement in the lower incisors, an immediate provisional is not advised due to the higher failure rates on very narrow implants.  We will often take the coronal portion of the tooth and bond it to the adjacent anterior teeth. At the second stage surgery the bonded tooth is removed, the second stage is completed and an essix appliance is fitted for the restoration process.

Here we have another case where I performed an extraction, implant, bone graft and provisional in one visit. This case is notable in that the provisional had to be made a bit shorter because of the patient's bite; I had to be sure that no pressure would be placed on the implant during the healing process.

This patient presented with extensive recession on the lingual of his mandibular anterior teeth. This is a difficult area to complete a CTG; many periodontists choose not to do lingual grafts because of the mobility of floor of the mouth, the tendency of patients to disrupt the graft, and the difficulty of access during surgery. That said, when done well, it is a helpful technique to prevent the loss of lower incisors. In this case, the patient healed very well and the teeth are now stable.

This patient had undergone a failed graft surgery at another practice, and was understandably reluctant to go through the process again. Fortunately, after 2 years of encouragement in our office, the patient agreed and we performed a connective tissue graft at #24 with excellent results.
