SFCCC is a network of nine nonprofit community health centers that serve over 11% of San Francisco’s population. We focus on the "big picture" of an ever-changing and competitive health care environment, promoting the well-being of our partner health centers so that their long-standing tradition of community-based health care is preserved. Our nine partner clinics operate from 21 sites strategical
ly located in neighborhoods with the greatest health disparities, allowing patients to connect with clinics that are accessible, speak their language, and understand their culture. Our partners offer service in over 13 languages and in multiple dialects. Our sensitivity to race, culture, and gender and sexual identity is core to our clinical practice. SFCCC partners keep people well, lowering health care costs by preventing unnecessary hospitalizations and reducing inappropriate hospital emergency room (ER) visits. In a recent study, multi-day hospital admissions were 64% lower for adult Medi-Cal patients served by a Federally Qualified Health Center (FQHC) than for adult Medi-Cal patients served by other providers. In addition, FQHC patients had 18% lower rates of ER visits and 4.9% lower 30-day readmission rates. Each year, our partners provide approximately 345,000 medical and dental visits to over 95,000 low-income San Franciscans, who would otherwise have nowhere else to go. Many patients of SFCCC partners struggle to survive financially – 60% are below 100% of the federal poverty level (FPL) and 78% are below 200%. Our partners employ over 600 workers and bring over $90 million in revenue to San Francisco to support services for our client populations. Less than 15% of our funding comes from the city.
