We’re thrilled to introduce Anna Larson, PT, DPT, our new physical therapist here at Femina Physical Therapy! Anna’s journey in healthcare began with a Bachelor’s in Public Health and a passion for nonprofit work, before she found her calling in physical therapy.

With a Doctorate in Physical Therapy from the University of Washington and a diverse clinical background, Anna brings a wealth of expertise to our team. Her dedication to pelvic floor rehabilitation is evident through her specialized training and continued education, including courses from Herman and Wallace Pelvic Rehabilitation Institute and the American Physical Therapy Association.

Anna’s holistic approach to patient care focuses on evidence-based practice and individualized treatment plans. She’s committed to empowering her patients through their pregnancy and postpartum journeys, offering support for a range of conditions from low back pain to pelvic pain disorders.

Blog Post: Endometriosis & Central Sensitization

Endometriosis is characterized by endometrial-like lesions that grow outside of the endometrium. People with endometriosis have symptoms such as: chronic pelvic pain, dysmenorrhea, abdominal pain, and more. About 20% of patients with endometriosis treated through surgical excision or ablation continued to experience chronic pelvic pain and other endometriosis symptoms. Moreover, 70 to 80% of patients with chronic pelvic pain with confirmed endometriosis lesions do not have lesions in the areas where they experience symptoms.

Central sensitization is the excessive firing of the nociceptors in the central nervous system and eventually starts to, “amplify and perpetuate the perception of pain long after the initiating pathology resolves.” (Aredo, et al.) Eventually, patients start to experience allodynia (pain from a non-painful stimulus) and/or hyperalgesia (increased pain to a painful stimulus). Three processes may be responsible when it comes to chronic pelvic pain:

1. Viscerosomatic convergence: visceral (organ) input to the brain almost always includes nearby muscle and skin input via viscerosomatic convergence
2. Viscerosomatic reflex: both visceral and somatic nociceptors connect with interneurons in the spinal cord that can activate alpha and gamma motor neurons that innervate skeletal muscle
3. Chronic, repeated local pain stimuli may affect the hypothalamic-pituitary-adrenal axis leading to decreased cortisol levels and thus exacerbate pain.

Over time, dysfunction in the muscle and surrounding connective tissue via the viscerosomatic processes can lead to myofascial pain and trigger points. People with confirmed endometriosis often have trigger points in the abdomen and pelvic floor. It is no wonder then, that most people with endometriosis also have pain with s*x, constipation, and painful urination (however conditions of these organs can occur concurrently as well).

Endometriosis symptoms can vary so much from menstrual pain to frequent vomiting. These professionals are so helpful in helping manage your endometriosis symptoms:

Gynecologist: Can confirm a suspected diagnosis of endometriosis, educate on options, medications, and set you up with a surgeon that specializes in endometriosis excision surgery
Gastroenterologist: Can help relieve some of your digestive related concerns (bloating, painful defecation, etc )
Pelvic Floor Physical Therapist: Can help musculoskeletal issues related to endometriosis such as jaw pain, headaches, hip pain, low back pain and especially pelvic pain through education, manual techniques and individualized programs
Nutritionist/Dietician: Can help guide you through different types of modified eating to help relieve some of your digestive symptoms and pain symptoms (IC, SIBO, and anti-inflammatory diets)
Pain Management Specialist: Especially in the beginning, it is helpful to have medications that can decrease your pain so you are able to perform everyday activities and be able to be a student or employee without being in pain or without having to call in “sick.” There are various forms of medications such as suppositories, creams, antidepressants, and more.
Counselor/Therapist: We know that many people with endometriosis also have or develop anxiety and/or depression. Having a therapist part of your team can help you develop tools on how to cope with the pain during your recovery
Other holistic therapies: acupuncture, reiki, aromatherapy, and other types of holistic therapies are definitely helpful in promoting a more parasympathetic switch in your body that allows your body to heal

For most women ages 21-65, it’s generally recommended every three years. Women over 30 might opt for a five-year interval when combining Pap and HPV testing.
