Does adding an internal brace to ACL reconstruction improve outcomes—or just add cost?

New in : At 2-year follow-up, ACLR with suture tape internal bracing was safe and cost-effective, with comparable functional outcomes and no graft failures or reoperations in either group.

🔑 Key findings:

• No graft failures, reoperations, or complications in either group at 2 years

• Functional scores (Lysholm, KOOS, IKDC) comparable between groups

• SANE score slightly but significantly higher with internal bracing (95 vs 90, p = 0.012)

• Technique was additionally cost-effective through minimised implant use

New in : A novel arthroscopic indicator—the posterior sentinel sign on the long head of the biceps tendon—is significantly associated with supraspinatus tears that MRI may miss.

🔑 Key findings:

· Posterior sentinel sign: specificity 96.7%, PPV 95.5% for supraspinatus tears

· Anterior sentinel sign: specificity 100%, PPV 100% for subscapularis tears

· Both signs help identify occult anterosuperior rotator cuff tears during arthroscopy

· Presence of either sign should prompt a targeted search for hidden cuff pathology

Can low-dose oral corticosteroids improve recovery after TKA—safely?

This double-blind RCT found that 3 weeks of postoperative deflazacort improved pain, sleep quality, and inflammation—with no steroid-related adverse events.

🔑 Key findings:

· Significant reductions in VAS pain scores at 1 and 3 months
· Better sleep quality at 3 months (PSQI: 9.44 vs 10.42)
· Lower ESR, CRP, and IL-6 at 3 months (all p < 0.01)
· No steroid-related adverse events

New case report in : An isolated dorsal dislocation of the intermediate cuneiform in a 32-year-old woman that failed closed reduction due to soft tissue interpositioning, requiring open reduction and internal fixation.

🔑 Key takeaways:

• Advanced imaging (CT/MRI) is essential to guide treatment planning

• Closed reduction is often unsuccessful—be prepared to convert to open reduction

• Two lag screws achieved stable anatomical alignment and full return to activities

• Monitor neurovascular status closely given the risk of foot compartment syndrome

Autograft or allograft for ACL reconstruction—does it matter at five years?

New in : In 480 ACL reconstructions, BTB allografts carried more than double the failure risk of autografts and significantly lower return-to-sport rates.

🔑 Key findings:

• Graft failure: 20.5% (allograft) vs 9.3% (autograft)

• 5-year graft survival: 77.4% vs 91.2%

• Return to preinjury sport: 60.9% vs 82.1%

• Allograft an acceptable option in lower-demand or recreational athletes

Does adding ALL reconstruction to ACL surgery reduce residual rotational laxity—without compromising outcomes?

Combined ACL + ALL reconstruction resulted in less residual pivot shift with comparable functional outcomes at short-term follow-up.

🔑 Key findings:

· Residual pivot shift: 4.8% (combined) vs 28.6% (isolated ACL)

· IKDC and Lysholm scores improved significantly in both groups

· No re-interventions or graft re-ruptures in either group

A tibial spine fracture with a hidden second injury—have you seen this before?

New case report in : A 23-year-old female with simultaneous ACL avulsion at the tibial insertion and proximal detachment at the femoral origin—a pattern the authors propose calling "Bipolar ACL Injury.

🔑 Key takeaways:

· Tibial avulsion fractures may conceal a concomitant femoral-sided ACL tear

· MRI is critical—tibial fixation alone may be insufficient in these cases

· Primary ACL reconstruction with semitendinosus autograft achieved excellent outcomes

· Surgeons should be prepared to change strategy intraoperatively

New case report in : A 12-year-old with 6 months of recurrent knee effusion after a glass injury—normal X-rays, inconclusive MRI, negative cultures.

What was missed?

Retained intra-articular glass fragments, identified only on thin-slice CT, and successfully removed arthroscopically.

🔑 Key takeaways:

· Normal radiographs do not rule out intra-articular foreign bodies after penetrating trauma

· Thin-slice CT is essential when MRI is inconclusive and effusion persists

· Arthroscopic retrieval with a shaver-mounted collector enabled complete removal

· Patient returned to volleyball pain-free at 6-month follow-up

How does periprosthetic joint infection after TKA in sub-Saharan Africa compare globally?

A two-centre cohort study from southern Nigeria found a PJI incidence comparable to high-income countries, and identified key modifiable perioperative risk factors.

🔑 Key findings:

· PJI incidence: 2.6% across 392 primary TKAs
· Staphylococcus aureus was the most common causative organism
· Patients treated for PJI still demonstrated meaningful functional improvement

Does pelvic width influence patellofemoral pain in Asian patients?

A study of 100 patients with anterior knee pain found no significant link between pelvic width and patellofemoral alignment—local knee anatomy is the real driver.

🔑 Key findings:

· No association between pelvic width and patellofemoral alignment or tracking

· No clinically meaningful sex-based difference in pelvic width

· Female patients had a higher prevalence of shallow trochlear morphology

Can hip dysplasia actually improve access to posterior CAM lesions? 🦴

New in JISAKOS: In dysplastic hips with a rare pistol-grip deformity, an arthrotomy via the Hueter approach allowed complete correction of large posterolateral CAM lesions prior to PAO.

🔑 Key findings:
• Posterior undercoverage facilitated access to the CAM deformity
• 100% elimination of the pistol-grip deformity
• Combined procedure increased operative time vs isolated PAO
