We got a shout-out in the AARC News!

Capital Health in Pennington, NJ is seeing "a decrease in LOS" since starting use of ENDOCLEAR systems. šŸ˜€

Five years ago the CDC replaced its ventilator-associated pneumonia (VAP) surveillance definitions with ventilator-associated event (VAE) definitions.

What happens when a vent circuit is disconnected to change a suction catheter or perform a bronchoscopy? Well... if you use a standard Closed Suction System, it's not good! (as you can see from the video). However, the endOclearĀ® LiberatorĀ® artificial airway maintenance system is designed to prevent derecruitment when changing catheters or performing bronchoscopy (or BAL) – a far better choice for patients with for whom even momentary disconnection would be a problem (i.e. ARDS patients). Not only does the Liberator system help keep the ETT clear and reduce artificial airway resistance, it can prevent loss of PEEP! Another illustration of how we are changing the standard of artificial airway maintenance for the better…

A recent evaluation - ETT extubated just after 24 hours

Pt was a young guy who was found down at home and intubated for 24 hours while under the trauma surgeons (intensivist) care. The physician spoke out at a meeting that morning saying that ā€œbuild up could not occur in under 24 hoursā€ (a skeptic on the benefits of ETT clearing). When his pt was extubated the RT grabbed the tube and gave it to us at the site and we made this video of the Liberator ETT clearing system to show him. After viewing the video, the physician contacted the RT Coordinator later in the day to tell him that he was now a ā€œbelieverā€.

Changing the world, one patient – one RT – one physician at a time… On to the next…!

Why we do what we do... Each patient's tube (pictured below) had just undergone current "standard of care" closed suctioning. All this stuff was removed by the endOclear® Liberator® balloon wiper immediately after. See what you are missing? 🤢

Q: Why is it ok to leave a closed suction system (CSS) in place for a week or more? Why does VAP or length of stay not increase compared to 24 or 48 hour change protocols? A: Because the CSS is not the source of the problem... What we know..http://www.endoclearinc.com/files/Download/Post-03-2015-10-13.pdf

A. Interventions with no impact on VAP rates or patient outcomes and unclear impact on costs
1. Closed endotracheal tube suctioning systems (quality of evidence: II).
a. Meta-analyses have found no difference in VAP rates, duration of mechanical ventilation, intensive care length of stay, or mortality between patients randomized to open versus closed endotracheal suctioning systems...http://www.endoclearinc.com/files/Download/Post-02-2015-10-07.pdf

What is the efficacy of suction to clear the endotracheal tube? For something that is the standard of care for clearing mucus and secretions from the endotracheal tubes of intubated patients and has been in use since the beginning of mechanical ventilation, that simple question stumps most professionals in the field and has a surprising answer. Follow our upcoming posts during the month of October to learn more about this little known problem and what we are going to do to address it.

Tenacious secretions are not removed by suction and remain on the walls of endotracheal tubes proving a home for bacteria and fungus. As you can see in the video, endOclear(R) removes what suctioning leaves behind.
