Mt. Hood Senior Living is seeking an outstanding, compassionate RN to manage and oversee the Health Services Department at our new RCF and Memory Care community opened on 03/20/2023!
The Health Services Director is responsible for supervising all care staff; coordinating all personal care services provided to residents by personal care staff; promoting the health and well being of each resident; and reporting observed changes in resident condition or health to the Executive Director, RN and resident’s responsible party. Health Service Director is responsible for maintaining all Clinical Compliance in regards to Oregon Rules and Regulations.

Clinical Oversight:
The Health Service Director performs a thorough health assessment on all new residents upon admission to the community. The Health Services Director is responsible for monitoring the health status of all current residents through assessments, evaluations, and care conferences scheduled quarterly or more frequently as required. Resident assessments, evaluations and care plans will be entered into the approved software document by the Health Services Director will monitor compliance with those care plans by all staff. This monitoring includes initiating actions as required in improving their health status and reading of all alert charting from daily report. This monitoring is to include but not limited to: medication side effects, activities, meals, complaints, cognition and behaviors, ADL’s and special requests. Using this information, the Health Services Director will coordinate and attend resident Care conferences and service plan reviews with a focus on assuring that the service plans are developed to meet the resident needs. In addition, the Health Services Director will react immediately to suspected resident abuse including a thorough investigation and notification of appropriate parties.
Supervisory/Administrative Duties:
The Health Services Director supervises Med-Aides and the Caregivers at Mt. Hood Senior Living. This responsibility includes hiring, coaching, orientation, scheduling and training of staff. Provide employee evaluations per Mt. Hood Senior Living policy and procedures and communicate all employee relations’ issues with the Executive Director. The Health Services Director performs all necessary verbal and written disciplinary actions required of supervised staff, and ensures documentation and process meets Mt. Hood Senior Living policies and procedures. The Health Services Director attends daily management meetings, staff meetings, and other meetings as delegated by the Executive Director such as: support group meetings, resident council meeting, and attendance at family night gatherings. The Heath Services Director working with the Executive Director will conduct facility audits as required of current operational systems including review of swing and night shifts to monitor staff compliance with policy and procedure and to ensure community security. The Health Services Director and Executive Director will review all incident reports, conduct investigations and report/plan corrective action as required.
Continuous Quality Improvement:
The Health Services Director working with the Executive Director will assist in ensuring there is a Continuous Quality Improvement (CQI) activity in the building. Working with the Executive Director, assist in identifying and set priorities in resolving service related issues, and those CQI activities within the community meet Mt. Hood Senior Living goal of quarterly meetings, and document plans to improve quality. The Health Services Director will provide additional areas of CQI focus through clinical audits performed in the community as required and requested by Mt. Hood Senior Living.
Compliance with Rules and Regulations:
The Health Services Director will support the Executive Director in maintaining compliance with all local, state, federal and licensing rules and regulations associated with the licensure of the building as they pertain to clinical and risk management activities. This includes HIPAA, OHSA, and State Survey issues, etc that apply to the specific location of the property
Safety Compliance:
The Health Services Director is responsible for ensuring that all employees work in a safe manner and that unsafe actions are to be managed. Compliance with Safety Committee standards is required. Should workplace injuries occur, the Executive Director would be responsible for following all procedures for treatment, investigation, return to work, and management of the injured worker, communication with care provider and Mt. Hood Senior Living.
Team Effectiveness:
The Health Services Director is responsible for fostering teamwork with co-workers, showing initiative in overcoming obstacles, takes actions to assist others beyond job responsibilities, and has a positive impact on co-workers, residents and visitors.

EDUCATION & LICENSING
Required: Licensed or Registered Nurse, State specific accreditation or license required.
EXPERIENCE
Must have proven management experience and ability to manage staff. Minimum three years experience in assisted living, Alzheimer’s or Memory Loss care.
Experience in Residential Care Management, Clinical Director, or Risk Management
Experience with Microsoft Word and Microsoft Excel
