Homeless service providers came together at the newly opened Sunrise Villa Apartments in Paso Robles on September 22 for the very first North County Homelessness Network meeting, hosted by the City of Paso Robles.

This inaugural gathering, led by the City’s first-ever Homeless Services Manager Ashlee Hernandez brought forward meaningful conversations around shared needs, collaboration, and opportunities to strengthen support for our unhoused neighbors.

The Wisdom Center’s Housing & Homelessness Director, Lupe Terrones, and Senior Housing Navigator, Eva Tagle, were in attendance, joining other local partners in the discussion and tour of the new affordable housing community, a hopeful reminder of what’s possible when partnership and purpose come together.

Last week, The Wisdom Center/California Care Management had the opportunity to tour the Santa Barbara Rescue Mission and witness the incredible work they’re doing to restore hope and rebuild lives. From offering shelter and meals to guiding individuals through recovery and job readiness, their impact is truly inspiring.

Today we had the pleasure of presenting our programs: Adult Day Health Care (ADHC), Enhanced Care Management (ECM), and Housing & Homeless Services (HHS) at staff meeting.

At The Wisdom Center, we provide:
✅ Adult Day Health Care (ADHC): Medical, therapeutic, and social services to help older adults and adults with disabilities stay healthy, independent, and connected.
✅ Enhanced Care Management (ECM): Whole-person care coordination for members with complex health needs connecting them to doctors, specialists, behavioral health, and social supports.
✅ Housing & Homeless Services (HHS): Helping individuals experiencing or at risk of homelessness find housing stability, case management, and supportive services.

Today The Wisdom Center/California Care Management attended the San Luis Obispo County Commission on Aging meeting!

We heard a powerful presentation on Su***de Prevention in Older Adults from:
Brita Connelly – Behavioral Health Program Manager, County of SLO Behavioral Health
Roberto Cueva – Program Manager, Central Coast Hotline (TMHA)
Nate Johnston – Central Coast Veterans Helping Veterans

At The Wisdom Center, we know that older adults often face unique challenges such as isolation, declining health, and loss of independence. These factors can deeply impact mental health, which is why we believe prevention starts with connection, compassion, and access to supportive services.

Today we joined the PACT Meeting at Restorative Partners to share about our Enhanced Care Management (ECM) and Housing & Homeless Services (HHS) programs.

These programs provide vital re-entry services, helping individuals navigate health care, housing, and community supports as they transition back into the community.
