Fee- for-service local PRI and Screen assessments are available in the Saranac Lake school district and Lake Placid by a qualified RN certified assessor! These assessments are mandatory in NY state for placement in a skilled nursing facility from home for short term rehabilitation or long term care and are the first step in getting your loved ones the appropriate care needed. $300 for both assessments that are good for 90 days (with some exceptions).
