As vestibular professionals, we routinely assess patients experiencing Benign Paroxysmal Positional Vertigo (BPPV).

Are you experiencing dizziness, blurry vision when moving your head, or feeling off-balance? It might be your Vestibulo-Ocular Reflex (VOR) acting up! Your VOR keeps your vision clear and stable when your head is in motion—think reading a sign while walking.

If this system isn't working right, you can experience symptoms like oscillopsia (where the world seems to jump or bounce), dizziness, and imbalance. The good news? Physical therapists can help!

Specialized tests, such as Dynamic Visual Acuity (DVA) and the Head Impulse Test (including the video version, vHIT), can identify and measure VOR problems.

A core part of vestibular rehabilitation is VOR x1 training, also known as gaze stabilization exercises. This involves specific exercises where you focus on a target while moving your head. Therapists carefully adjust things like:

Target: Size, type (like letters on a chart), distance, and background.
Head Movement: Speed (sometimes aiming for specific frequencies like 1.3 Hz or around 150-240 BPM), and the arc of motion (around a 40-degree arc is often mentioned).

Body Position & Surface: Start seated and progress to standing on different surfaces (even foam pads!) to challenge your balance.
Duration & Frequency: Typically, exercises are done for 1-2 minutes per set, multiple times a day, aiming for a total daily exercise time (e.g., at least 12 minutes/day for acute issues, 20 minutes/day for chronic).

The goal is to help your brain adapt and reduce those troublesome symptoms! If you're experiencing these issues, talk to a physical therapist specializing in vestibular rehabilitation.
