We will be closing our current Sartell location and the Country Manor "Quick Clinic" to move into our new clinic. The Foley location will remain open, but under new ownership.

Did you know? Integracare offers Chronic Care Management (CCM) for patients with chronic conditions!

CCM provides patients with a care team and a dedicated point of contact at Integracare to be in correspondence with regarding care for chronic conditions.

What Does CCM Provide?
After enrolling in CCM, you will be assigned a care team that will be in regular contact with you. They will provide information and encouragement to help you stay on track and manage your chronic conditions.

Your CCM team can also help connect you to valuable resources in our community that you may be eligible to participate in.

Why Is CCM Beneficial?
Your CCM team coordinates care between your regular office visits, pharmacy, and any specialists you have. Regular CCM check-ins are completed over the phone.

How Do I Sign Up For CCM?
