Respiratory Disorders of the Pleural Space

The visual illustrates four conditions where substances abnormally accumulate in the pleural space, which is the thin area between the lung and the chest wall.

1. Pleural Effusion
The accumulation of FLUID in the pleural space.

Mechanism: Fluid (which is not blood or pus) builds up, often due to conditions like congestive heart failure, pneumonia, kidney disease, or cancer.

Effect: The presence of fluid compresses the lung, making it difficult for the patient to breathe fully.

2. Hemothorax
The accumulation of BLOOD in the pleural space.

Mechanism: This is typically caused by trauma (e.g., a car accident, stab wound) or sometimes due to surgery or a ruptured blood vessel.

Effect: The blood puts pressure on the lung, leading to partial or complete lung collapse, and can also cause a significant loss of circulatory volume (shock).

3. Pneumothorax
The accumulation of AIR in the pleural space, causing lung collapse.

Mechanism: Air enters the pleural space, often through a chest injury or a rupture of a small air sac on the lung surface (a bleb).

Effect: The air pressure causes the lung to recoil and collapse (pneumo = air, thorax = chest), leading to sharp chest pain and shortness of breath.

4. Empyema
A collection of PUS in the pleural space.

Mechanism: This is an infectious process, usually developing as a complication of pneumonia when the infection spreads from the lung into the pleural space.

Effect: It is a serious infection that causes fever, chest pain, and difficulty breathing, requiring drainage of the pus and antibiotic treatment.
