!!! Exciting Announcement: New Membership-Based Primary Care at SKA Health and Wellness!!!
Hi everyone! We are excited to share that SKA Health and Wellness is now offering a Direct Primary Care (DPC) membership program! DPC is a modern patient-centered model that allows you to receive primary care through a simple monthly membership fee instead of using insurance. There are no copays, no surprise bills, and no insurance involvement.
With a DPC membership, patients receive unlimited routine and preventative care, chronic care management, sick visits, and ongoing communication directly with your provider/care team whether in office, by phone or text, or through telehealth. Members also have access to discounted vitamin injections, discounted skin-care, in-office procedures, wellness labs, and coordinated care with other providers.
This program is a great option for anyone who wants more personalized, flexible care, predictable costs, and easier access to their provider. We have several membership options available for individuals, couples, and families. We have non-member options as well.

This initial visit is for us to meet you and to determine that our clinic can meet your needs. If you do not enroll there will be no charge for this initial consult. If you want to enroll the first month payment and any applicable enrollment fees will be due the day of the visit.
FAQs:
1. Do I still need health insurance?
Yes. DPC replaces primary care only. You’ll still want insurance for specialist visits, medications, outside labs and imaging, ER visits, and hospitalizations.
2. Can I use my insurance for office visits?
No. DPC does not bill insurance. Your monthly membership covers all routine and preventative primary care services.
3. Are labs included?
Your annual wellness labs are included with enrollment when applicable. Any additional labs or specialty tests are offered at discounted cash prices.
4. Is imaging included?
No. X-rays and other imaging services are not included but can be ordered and performed at the facility of choice.
5. Can I join if I have Medicaid or Medicare?
While we do not accept Medicaid patients, Medicare beneficiaries can enroll in the DPC program but all services are private-pay and not covered by Medicare.
6. What if I rarely go to the doctor?
DPC is designed for flexible, unlimited access even if that only means a few visits each year. Many members value the peace of mind of having quick and easy access, longer visits, and ongoing communication.
7. What’s included in the membership?
DPC membership includes routine and preventive care, chronic disease management, sick/acute visits, communication by phone/text/telehealth, yearly wellness labs, discounted skin care, discounted vitamin injections, select in-office procedures, and coordination of care.
8. How do payments work?
Membership fees are billed monthly. An initial enrollment fee may apply for some memberships. A 3% fee applies to credit or debit card transactions but checks can be used to avoid this fee. (Auto-draft is preferred)
9. How do I enroll?
You will be scheduled for a DPC Membership Consultation where your provider will go over your needs and goals of care and help you fill out any necessary paperwork.
10. Can I cancel my membership?
Yes. There is a 3-month minimum commitment and after that, you can cancel at any time with written notice.
11. What if I’m not a member and want to schedule a one-time visit?
SKA Health and Wellness welcomes both members and non-members at cash pay rates. Non-member rates are based on time spent and condition treated. Payment is due at time of service.
