August Birthday Party! Unfortunately, no residents who were born in the month of August attended, but other residents enjoyed the party for them. One staff member participated, since she will be celebrating her birthday at the end of this month. Cupcakes, ice cream and other refreshments were provided!!

Positive Outcome for Samuel Jones!
Mr. Jones was admitted on 3/27/25 for skilled nursing and rehab services after a right knee infection. During his stay, he worked hard in therapy and was able to walk about 50 feet with contact guard minimal assistance and transfer with min. assistance. He still required moderate assistance with his Activities of Daily Living. Unfortunately, he had a setback due to increased pain and swelling of his legs. he was unable to walk and was using a sit to stand life for transfers. Therapy and medical staff worked together to put a plan in place to address the pain and the edema, and he soon was back on track. With an extended rehab stay, he was able to increase his walking distance to 200 feet with supervision, transfer with mod. independence and go up/down five stairs with supervision. He was able to toilet with mod. independence and required supervision contact guard with lower body dressing. Therapy completed family training, and he was able to return home with home health services and assistance from his significant other. He chose Lofland Park from his previous stay here and was motivated to get stronger and go home. When asked what his favorite thing about his stay was, he said that the Breakfast Group in therapy was his favorite. He was able to make his own breakfast and sit and talk with other people in the group and felt "normal". Lastly, he stated, "I couldn't have gotten home without therapy". Congratulations Mr. Jones on all of your hard work; we are very happy for your return home!

Positive Outcome for Barbara Blew!
Ms. Blew was admitted to Lofland Park Center for skilled nursing and rehab services on 4/3/25 after an ORIF for a right femur fracture due to a fall at home. When Ms. Blew was admitted, she was non-weight bearing to her right lower extremities and using a hoyer lift for transfers. She was maximum to dependent for her Activities of Daily Living. Working hard with therapy, she was able to use a sliding board but still required mod-max assistance due to right should pain. Therapy consulted with the physiatrist onsite, and Ms. Blew was able to get an injection to her right shoulder to help manage her pain. With decreased pain and continued therapy, Ms. Blew was able to do sliding board transfers with min. assistance but still required max. assistance for ADL's and toileting due to being NWB on her right lower extremities. On May 5th, she was upgraded to weight-bearing as tolerated. She was motivate to improve her transfers so she could go home. She worked hard with therapy and able to do a stand/pivot transfer with contact guard, and required min. assistance for lower body ADL's and toileting and was walking 40 feet using a rolling walker with contact guard. Family training was completed, and Ms. Blew was discharged to home with home health services and assistance from her granddaughter. Lofland Park was where she was sent for therapy, and she was motivated to get stronger and return back home. When asked what her favorite part of her stay at Lofland was, she replied, " the staff are friendly". Congratulations Ms. Blew on all of your hard work; we are very happy for your return back home!
