Tried to approach each side differently for these horizontal impactions. You’ll see in the video what I could have done differently but both teeth were removed uneventfully.

Peripheral Giant Cell Granuloma in a healthy 20-something person.

• Benign reactive lesion
• Occurs only on gingiva / edentulous ridge
• Red–purple or blue nodular mass
• Sessile or pedunculated
• May bleed easily
• Usually < 2 cm
• Associated with local irritation

DIFFERENTIAL DIAGNOSIS

• Pyogenic granuloma
• Peripheral ossifying fibroma
• Irritation fibroma
• Hemangioma
• Metastatic lesion (rare)

TREATMENT

• Complete surgical excision
• Curettage to periosteum / PDL
• Eliminate local irritants

Recurrence: ~5–10%
Prognosis: Excellent
