Many people are nervous they will look “stretched” or that their mouth will look abnormal after a facelift. This typically results from poor surgical technique and a failure to understand and fully address all of the layers involved in facial aging. Fortunately this situation is completely avoidable and I have never had a patient tell me they look stretched or “windswept” after a facelift. A well done deep plane facelift should be natural and once the scars have faded nobody should be able to tell you had surgery. You should look exactly like yourself, just a more rejuvenated version of it!

This patient in her 40’s wanted to address what she called her “double chin.” She opted for a deep plane neck lift and is now 1 year out from her procedure. I love seeing how my results hold up down the road. This patients neck looks just as defined at 1 year as it did at 3 months which is exactly what I hope for all of my results!

If you are planning on having a facelift in the next few years there are a few aesthetic treatments that can make your surgery more difficult and potentially more risky:

1) Heat or ultrasound-based skin tightening treatments like thermage or ultherapy

2) Biostimulatory injectables like sculptra or radiesse to the cheeks or jawline. Injections in the temples are typically not problematic

3) Thread lifts - in addition to being very ineffective, they create scar tissue

Treatments that you can and should continue include good skin care (including retinol), laser treatments, microneedling, neuromodulators (like Botox)

Dermal fillers to the lips don’t cause problems (unless you are also interested in a lip lift) and can enhance a rejuvenation surgery. A conservative amount of subtle filler to the cheeks, temples, jawline, chin is usually not an issue. However, if you know you want surgery soon and are interested in fat grafting as part of your facelift it’s typically best to just avoid placing much filler which can “muddy the waters” and might have to be dissolved if fat grafting is planned.

It’s also best to avoid under eye filler if you are thinking about facial rejuvenation surgery. Usually this area is best treated with fat grafting and I will typically dissolve under eye filler if it is present before surgery.

Many patients wonder if we will have to break your bones during rhinoplasty. The answer is that it depends on the goals of the rhinoplasty and each patient’s anatomy, but the good news is that this is a lot less scary than it sounds. If we do perform osteotomies (breaks/cuts in bones) you may have a little worse bruising but it doesn’t make the pain any worse so it’s not something to be afraid of as a patient!

This woman in her 50s traveled to me from out of state for upper eyelid rejuvenation. She felt like her eyes looked tired and concentrated, even after a great night of sleep.

You can see that she has eyelid hooding that extends beyond the corners of her eye. This happens when there is brow heaviness in addition to eyelid heaviness and an upper blepharoplasty alone would not have been able to fully resolve her hooding.

You can see that her upper face just looks more relaxed and inviting. The goal of a brow lift is NOT to lift the brows as much as possible (which can make someone look surprised) but to restore a youthful brow shape, slightly lift the tails of the brow (which drop down as we age), and make the EYES look relaxed and inviting. She is absolutely thrilled with her results, as am I!

This woman in her 60s wanted to look as vibrant and energetic as she felt inside. The goal was not to change the way she looks but to restore some of her natural beautiful features that had been lost with time.

We performed a deep plane face and neck lift to tighten her jawline, lift her jowls, and restore her neck contour back to what it used to be. A lower blepharoplasty (lower eyelid lift) was used to remove her eye bags and we used a small amount of fat grafting to soften the transition from her cheek to her eyelid. A conservative browlift slightly elevated her brows and most importantly softened the harsh and concentrated look that she had developed between her eyebrows. Finally we performed a full face chemical peel to rejuvenate her skin and help with all of the sun damage and pigment issues she had developed over the years.

She is 3 months out from surgery and absolutely ecstatic about her results!

A septoplasty is a great tool to help nasal breathing in patients with a deviated septum. However having a prior septoplasty definitely makes getting a rhinoplasty more difficult in the future. As we age many things happen to our lips - the upper lip skin gets long and flat, the red part of the lip loses volume and shows less, and the corners of the mouth can descend creating a “frown” look. This patient (and aesthetic injector in Oregon) was bothered by all of those aging signs and is now 11 months out from a sub nasal lip lift and corner of mouth lift and is thrilled with her new lips!

The sub nasal lip lift is done with a small incision at the base of the nose and shortens the distance from the base of the nose to the upper lip back to a more youthful proportion. In a youthful lip when the mouth is slightly open you see just a tiny bit of the upper teeth, which is often lost as the upper lip gets long with age. You can see in the second photo how this tooth show has been restored. Although no volume was added to the upper lip it looks slightly fuller and has more of a pout after the lip lift. Some patients go on to supplement their lip lift with a little bit of filler to add volume which compliments the lift beautifully.

The corner of mouth lift helps lift the downturned sides of the lip that create a sad or frown expression. It also helps reveal a subtle amount of the sides of the lip that get hidden as the upper lip gets long and rolls inward with age.

A lip lift is a short outpatient procedure that can be done without general anesthesia. Patients can expect swelling and bruising to their lips that lasts about 1-2 weeks. The scar under the nose is pink at first but fades with time and is typically very difficult to see with time.
