The Skinny on GLP-1 agonists
Ozempic and Mounjaro

These popular weight loss drugs contain hormones that slow stomach emptying, give people an early sensation of fullness, stimulate the pancreas to secrete insulin and decrease which lowers blood sugar levels. Well designed for diabetes, it was found that people lost significant amount of weight due to its effects on the stomach, making people feel full earlier and slowing down emptying of the stomach. It also has interesting effects of decreasing the desire for alcohol use. There are too medications made by two different drug companies, semaglutide and terzepatide. Terzepatide has superior perfomanve due to the fact that it has two hormones that work together instead of one like semiglutide. Common side effects are heartburn, constipation, diarrhea, nausea. From my experience terzepatide has fewer side effects.

I have managed this drug in literally hundreds of people, and I have seen people “yo-yo“ on this medication. My biggest reservation on using the drugs is rapid weight loss without a short term plan to get to a goal weight and then have a plan to maintain it, getting off the drug . I have seen several people stay on the drugs or extended periods of time because we get addicted to easy weight loss and maintenance. This is exactly what big Pharma wants. I believe it is detrimental to your health and your wallet.

The cost of these medications has come down to approximately 50% of what their introductory costs were because of the compounding black market. I do not recommend buying compounded medications because you don’t know where they come from.

With the considerable discounts from the pharmaceutical companies, they still range in the neighborhood of $350-$500 a month, a pretty penny!

My approach to management is for diet, exercise, and various lifestyle changes with an end point of getting off the medication’s, maintaining your weight, and enjoying a healthy lifestyle at a lower weight without medications.
