Dear GVP Village,
To begin, I would like to thank you all for the trust you have given me over these past 4 years as your pediatric provider. Taking care of you and your children has been an honor for my staff and me. Your continued wellness and health are priorities for us.
I am writing today to inform you that after much prayer and thought, I will be closing my practice due to multiple reasons. I have had some medical issues arise and family commitments and responsibilities that simply cannot be overlooked. My last day to see patients in my office will be on Wednesday, May 24, 2023. I will be available until that date for your care.
It is important that your child/children continue with appropriate medical care; therefore, you should establish contact with another provider as soon as possible. In order to help you find another provider, we have contacted the pediatric providers and family practice providers in our tri-county region and all except for 1 are taking new patients and have multiple insurance options. Our best advice is to contact your insurance provider and see who is available for your situation.
You may obtain a copy of the HIPPA medical request form from our website at WWW.GVPediatrics.com or can call or message our office with your email to have one sent to you! Please complete this form and return it as soon as possible. On receipt of this signed form, my office will forward a copy of your child’s medical records to you or the provider you designate. Please rest assured that our office will remain available during the next several weeks to consult with your new provider to promote a smooth transition and continuity of care.
After the close of my practice, signed requests for copies of medical records can be sent to GeronimoVillagePeds@gmail.com and records can be accessed through that process with the completed signed form. You can access a copy of the form requesting medical records on our website at WWW.GVPediatrics.com under FORMS.
If you have not selected a new provider by the time of my practice closure, you can obtain acute, critical or emergency care by seeking care at 1. Guadalupe Regional Medical Center ER or Urgent Care located in Clear Springs 2. Resolute Hospital ED 3. Christus Santa Rosa Urgent Care at Creekside or Christus Santa Rosa ED in New Braunfels or San Marcos, Tx.
