Lutheran Community Home houses 116 beds dually certified for both intermediate and skilled levels of care. Our skilled rehabilitation program offers a wide range of services to maximize functional independence and facilitate a safe discharge home. Our Secure Memory Unit offers assistance for older adults suffering from dementia or Alzheimers Disease. Evergreen Way houses 33 beautiful assisted living units and our independent living options include the Garden Homes and Church Avenue Manor apartments.
