🎉Our Shenandoah Health Center is now open!

Thank you to everyone who supported operations and helped ensure continuity of care during the temporary closure. We truly appreciate your flexibility, teamwork, and continued dedication to our mission during this time.

With services starting up again at Shenandoah Health Center, we’re happy to welcome a new member to our team. Yalixsa Then, CRNP, joined PHN in September 2025 and will serve patients at the Shenandoah location. She earned her BSN from Long Island University, Brooklyn, and her MSN from Lehman College, Bronx.

Yalixsa has a solid clinical background and always puts patients first. We’re sure she’ll be a valuable part of the Shenandoah team and help us care for our community.

🌟 Join us in welcoming Ava Homan, CRNP, to the PHN family!
We’re happy to welcome Ava Homan, a Certified Registered Nurse Practitioner, who joined Primary Health Network in October 2025. Ava offers family medicine services and brings valuable experience and a real dedication to caring for patients.

She earned her Bachelor of Science in Nursing from Clarion University of Pennsylvania and her Master of Science in Nursing from PennWest University.

We’re glad to have her join us as we work to make compassionate, high-quality family medicine more available in our communities.

🌟 Thank You for Making the 2025 “Swinging Fore Literacy” Golf Outing the Best Yet! 📚

What a day! On October 6, our PHN Charitable Foundation celebrated their “Swinging Fore Literacy” golf event. Thanks to their amazing sponsors, donors, golfers, volunteers, and golf course staff, they raised over $62,000 with 164 registered golfers. This year’s outing was their most successful ever, and it’s all thanks to YOU!

Your support helps them grow their Little Free Library and Reach Out & Read programs, and as a result of your generosity, they’re expanding their Caring for Our Neighbors program with new initiatives that support PHN employees’ educational advancement and provide direct support to PHN patients.

👟 Huge thanks to their 20-Year Platinum Anniversary Sponsor, Sharon American Legion Post 299, for helping them provide every golfer with a pair of golf shoes to commemorate this special milestone!

This year was extra special as they marked their 20-year anniversary. Together, they’ve:
✅ Donated over $707,000 to local organizations
✅ Raised $1.5M through 150+ events
✅ Awarded 221 scholarships
✅ Distributed over 3,300 books through Reach Out and Read
