Part-Time Direct Care Worker Positions Available in Pittsburgh- South Hills (South Hills, Brentwood, and surrounding areas).
Landmark Home Health is a nonprofit organization that believes in making a difference in the lives of those who need it most. We provide personalized, compassionate care to individuals who require assistance with their daily activities, enabling them to lead fulfilling lives in the comfort of their homes.
We support the Public Service Loan Forgiveness (PSLF). The PSLF program helps individuals in public service roles alleviate the burden of student loans.
Responsibilities:
• Assist clients with daily activities, such as bathing, dressing, grooming, and meal preparation.
• Perform light housekeeping, laundry and grocery shopping tasks.
• Maintain accurate records of services provided and any changes in clients' condition.
Benefits Include:
• Up to $400 sign-on bonus
• Personal time off
• 6 paid Holidays per year
• Paid orientation
• Paid travel time
• Monday to Friday daylight hours (normally 8:00 am- 4:30 pm)
• NO WEEKENDS, EVENINGS OVERNIGHTS OR HOLIDAYS

Qualifications:
• High school diploma or GED
• 6 months of Home care experience is preferred
• Must have reliable transportation and valid current driver's license

• Ability to follow care plans and maintain confidentiality.

Landmark Home Health is looking for Physical Therapists in Monroeville, Penn Hills, Verona, and surrounding areas. As a Physical Therapist, you will provide high-quality care to patients in the comfort of their own homes. You will play a crucial role in helping patients regain their mobility and independence, improve their quality of life, and promote overall well-being. Strong clinical skills, computer skills, and of course Home Health Care experience is not required but a PLUS!
We support the Public Service Loan Forgiveness (PSLF). The PSLF program helps individuals in public service roles alleviate the burden of student loans.

RESPONSIBILITIES:
• Assists the physician in evaluating and assessing the level of function of the patient and in developing a Plan of Care.
• Interprets findings to the physician, patient, family and the professional nurse.
• Assists in establishing rehabilitative plans and goals to reach the patients optimum functional level.
• Provides comprehensive physical therapy treatment utilizing rehabilitation procedures to improve the patient's level of functioning.
• Advises and consults with patient's family and the professional nurse in implementing and evaluating the Plan of Care.
• Revises the treatment plan as indicated, upgrades program based on patient's progress and participates in discharge planning.
• Provides supportive care to the patient, family and significant others and assists them to accept and adjust to realistic plans and goals based on the patient's physical limitations.
• Assists with the selection and/or modification of equipment, appliances and physical set up of the home to meet the patient's needs.
• Recognizes the teaching needs of the patient, family and significant others. Develops a teaching plan to meet those needs.
• Initiates interdisciplinary communication to assure the effective implementation of comprehensive treatment. Participates in periodic patient evaluation conferences.
• Prepares clinical progress notes and completes required documentation promptly, accurately, and concisely in accordance with Agency Policy, Medicare and other third party reimbursement regulations.
QUALIFICATIONS:
• Current Pennsylvania Physical Therapy License is required
• Must have two years of current therapy experience
• Valid current driver's license and auto insurance coverage is required
WORKING CONDITIONS:
• Normal working hours are Monday through Friday, 8:00 am - 4:30 pm
• Participates in rotating weekend assignment rotation.

We are hiring a Speech Language Pathologist - PRN to serve in the Allegheny County area. As a Speech Language Pathologist at Landmark Home Health, you will play a pivotal role in providing high-quality care to patients. We require strong clinical and computer skills. Home Health Care experience is not required but a PLUS!

Provides diagnostic and rehabilitative services to patients with speech language and/or cognitive disorders. These are provided under the direction of the physician, with the participation of other health team members, family, and significant others as needed.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
• Assists the physician in evaluating and diagnosing speech and language disorders and level of functioning by evaluating causes and test results.
• Participates in the development of the Plan of Care.
• Interprets findings to the physician, patient, family, health team members, and significant others.
• Provides comprehensive speech therapy in the patient's residence within the guidelines of the American Speech, Language and Hearing Association (ASHA).
• Demonstrates, teaches and supervises communication strategies to the patient, family, health care team and significant others to maintain/improve skills.
• Advises and consults with patient, family and other health professionals in implementing and evaluating the Plan of Care.
• Revises the Plan of Care as indicated, upgrades program based on the patient's progress and participates in discharge planning.
• Provides supportive care to the patient, family and significant others and assists them to accept and adjust to realistic plans and goals on the patient's physical limitations.

• Has access to electronic or manual protected health information by scope and responsibility. Will be required to adhere to the privacy practices as detailed in the Notice of Privacy Practices, privacy policies and procedures.
• Maintains Professional License by completing the State mandated Child Abuse CEU’s.
KNOWLEDGE, EXPERIENCE, LICENSURE REQUIRED:
• Masters Degree in Speech Pathology.
• Certified Clinical Competence (CCC) in Speech Pathology, certified by the American Speech and Hearing Association (ASHA) or eligibility for CCC (e.g. Clinical Fellowship year).
• Minimum of one (1) year experience as a Speech Language Pathologist under the supervision of a Certified Speech Pathologist.
• Comprehensive knowledge of speech language disorders, theory and therapy techniques and the ability to adapt them to home situations.
• Knowledge of current home health insurance reimbursement regulations.
• Currently licensed to operate a motor vehicle in Pennsylvania and necessary insurance coverage as mandated by the State.
• Maintains current CPR certification.
WORKING CONDITIONS:
• Normal working hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. or as schedule dictates.
• Maintains a safe driving record and a vehicle in dependable working order.

Lynette Tringes has been a nurse for 15 years. Her career with Landmark Home Health began 12 years ago as a visiting nurse. She transitioned to a supervisor position almost 5 years ago. The thing she likes the most about nursing is helping people work to achieve goals due to challenges that may not have been what they expected. She loves the flexibility and independence that homecare has offered. She feels she was led to nursing because she had worked in a care home at age 16 and always enjoyed talking and caring for the patients. She feels she has learned just as much from the patients as she has taught them. After many years of saying she was going to be a nurse and encouragement from her dad, she became the first nurse in her family. (She wonders if the encouragement from her dad was because she was selling lingerie at the time) She now has a younger cousin that is a nurse and she is encouraging her niece to become a nurse.

Jessica Wheeler has been a nurse for 12 years. She has been with Landmark Home Health Care for 2 months. The thing she enjoys most about nursing is helping others and knowing she is making a difference in their lives. She enjoys working as a home healthcare nurse because of the flexibility and the one on one time with patients. She chose a career in nursing because she wanted to be able to help others. Fun fact about her nursing career: She has worked every job as a nurse with her best friend from nursing school except Landmark. Karena Gohn has been a nurse for 3 years and employed by Landmark Home Health care for 11 months. She has always loved taking care of people. At a young ages she was drawn to nursing. She even played doctor and nurse when she was young. She has always been nurturing to others. Landmark Home Health Care is her first home health position. She was drawn to home health care because of the flexibility. She also enjoys having the one on one patient care, which enables her to give the patients all the care they need. Denise McGreal has been a nurse for 34 years. Of the 34 years, 19 have been in home health care. She has been the Clinical Manager for Landmark Home Health Care for 8 years. Denise enjoys helping people. She likes home care because you can give individualized care to the patients. She became a nurse because she likes to take care of people. Fun fact about her nursing career: She was such a horrible typer that she used to write Patho papers and Care Plans for other nursing students, and they would do her typing. It is a good thing her typing skills have improved as she has not found anyone at Landmark Home Health Care to type for her!!!!!

Rebecca Kennedy been a nurse for 14 years. She has only been with Landmark Home Health Care for 1 week, however we are very happy to have her as part of our team! She has been a home care nurse for 2 years. The thing Becky enjoys most about nursing is talking with and supporting the patients. She likes being able to be outside and relatively being on her own within home care. She became a nurse to help people. Ironically, nursing is her second career. Prior to nursing, she was a Linguist. For clarification, a Linguist studies languages and the patterns and sounds within languages, how they are the same and different from each other.
