Alteas Health VP of Clinical Operations Nic Steffen, Cardiac NP, presented at AdventHealth Shawnee Mission to the case management department on Navigating Transitions of Care for Cardiac and Pulmonology patients.

Nic's presentation highlighted the importance of strong collaboration across inpatient hospital care teams, SNF care teams, and specialty care providers like Alteas Health to ensure safe, high-quality care transitions for cardiac and pulmonary patients entering the SNF setting.
