Our Mission at Embrace Care Management LLC- We are committed to providing compassionate, dignified and ethical care; ensuring peace of mind for seniors and their families. Taking care of an aging loved one can be overwhelming but you don't have to go through it alone. Our dedicated Social Workers and Nurses are able to help answer the difficult questions. We are an advocate for your loved ones when family is not near or maybe family is having a difficult time making some decisions like "should Mom stay in her home?", "how do we handle my parents memory loss?". Embrace also offers Managed Care Companions who can assist with daily tasks such as grooming, bathing, meals, housekeeping, memory support, medication reminders and so much more.
