We are nearing the conclusion, friends! The final day for submitting record transfer requests, which our office will handle, will be Thursday, October 16th.

If you have ordered copies of your records, they are available for pickup Monday through Thursday 8-4 until October 23rd.

If you haven't yet heard, our office plans to close on October 23rd. If you happen to need records transferred, have your new eye care provider give us a call, fax, or email. After the office closes, there will be a records management company for the transfers. We have enjoyed caring for our patients and want you all to have a smooth transition!
