The FDA had a ruling today to remove black box warnings on BHRT so this is very exciting as the safety of BHRT is something we have known for years!

A meeting was held by an FDA expert panel on July 17, 2025, to discuss hormone replacement therapy (HRT) and its associated risks, particularly regarding the black box warning on estrogen-containing products.

Here's a summary of the key points:
Black Box Warning on Estrogen: All FDA-approved estrogen products for postmenopausal women currently include a black box warning about the increased risks of endometrial cancer, cardiovascular events (including heart attacks, strokes, and blood clots), and probable dementia.

Panel's Recommendations: The advisory panel urged the FDA to remove these boxed warnings from labels for vaginal estrogen products, arguing that there's a lack of evidence to support the warnings for localized vaginal estrogen and that they deter patients and providers from using these beneficial treatments.

Basis of the Warning: The black box warnings originated from a 2002 study by the Women's Health Initiative (WHI) which found an association between estrogen-progestin tablets and higher risks of certain cancers and cardiovascular disease.

Distinction Between Estrogen Types: Experts, like Dr. Heather Hirsch, highlight that the WHI study focused on conjugated equine estrogen and medroxyprogesterone acetate, and that it's scientifically inappropriate to generalize those findings to all forms of hormone therapy, including 17 beta estradiol, which is a naturally occurring human estrogen and binds differently.

Patient Impact: Many women are reportedly hesitant to use vaginal estrogen due to the black box warning, even for low-dose formulations which are considered safe and effective by major medical societies for treating conditions like vaginal dryness, painful s*x, and recurrent urinary tract infections (UTIs).

Need for Change: There's a strong call to remove the outdated black box warning and ensure that women have access to evidence-based care for menopausal symptoms.
