I want to inform you I will be leaving my practices located at 13460 S. Archer Avenue, next to Target Optical, Lemont, and 1111 Brook Forest Avenue, next to Target Optical, Shorewood, effective 6/19/21. My associates, Dr. Lanae Yong and Dr. Margaret Slowik, will also be taking opportunities elsewhere. It has been a great pleasure providing for your eye care.
