Esperanza is a bi-lingual peer run, community based wellness
center that strives to provide support to individuals who are experiencing mental health and substance abuse issues. Peer Outreach Support Team (POST)
The Peer Outreach Support Team encourages people to take the responsibility for their own wellness and recovery. We usually work with closely with an individual for approximately three-to-six-months, developing a partnership to create and support the individual’s own recovery goals. In addition to the important one-to-one support, POST workers run a variety of groups, such as twelve step recovery meetings and sessions that focus on wellness, employment, communication skills, and social activities. Intensive Family Support Services (IFSS) is a county-wide support program for families with relatives with mental illness. Throughout NJ, many families take an active role in caring for the relative with mental illness, which can be a difficult and challenging task. The Intensive Family Acute Care Program of Union County provides support to families living in Union County who have an adult family member who is experiencing or is at imminent risk of a psychiatric crisis. The Acute Care Clinician provides support, coaching, information about resources, and help navigating the mental health system. The goal is crisis intervention and advocacy to help families access the appropriate level of care. Trained Family Advocates are available on-site in the Psychiatric Emergency department at Trinitas Medical Center to share their lived experience and offer support to families during the evaluation process.
