Join our growing team and play a key role in supporting clients and families through the intake and care coordination process.

Key Responsibilities:

Answer multi-line phone calls and manage all client intake/referrals
Enter and maintain accurate data in HubSpot and WellSky
Complete intake paperwork and route for admission approval
Assist with staff scheduling and case coordination
Communicate with clients, families, and contracted agencies
Maintain case-management notes and report changes in client or service needs
Complete home assessments, intake paperwork, and insurance-related tasks
Participate in on-call duties as needed
Approximately 25% field work for home assessments
