Thank you to the amazing Care Coordination team at Chiricahua Community Health Centers for welcoming Valor Hospice Care for a ‘lunch & learn’ today.

We had great conversations about how hospice can support patients and families earlier in their journey through comfort-focused care, symptom management, and additional resources when they’re needed most.

Partnerships like this help ensure patients in Cochise and Santa Cruz counties receive compassionate, coordinated care every step of the way.

We’re grateful for the work Chiricahua does for our community every day and look forward to continuing to collaborate together.
