GRADE YOUR PCL TEAR

The Posterior Cruciate Ligament (PCL) is one of the key stabilizers of your knee. When it’s injured, the severity can vary — and so should the treatment.

Here’s how PCL injuries are graded on MRI:

Normal: Intact ligament with uniform signal and structure.

Grade I: Mild sprain — the ligament is stretched but not torn.

Grade II: Partial tear — some fibers are disrupted.

Grade III: Complete tear — the ligament is fully torn, often leading to significant instability.

DO KNOCK KNEES NEED SURGERY?

If that knee is severely angled — especially over 12 degrees — and you’re considering total knee replacement, you need to hear this first!

Here at Dr. P’s 3 Secrets:

1. A valgus (knock-kneed) knee puts the common peroneal nerve at risk during surgery. Straightening that alignment puts tension on the nerve — which can lead to weakness, foot drop, or nerve palsy after surgery.

2. The risk jumps dramatically at 12° or more of valgus.

3. In a large study, knees with ≥12° of valgus had an 8.5% chance of peroneal nerve palsy — compared to just 0.22% in more neutral knees. That’s a nearly **39-times higher risk**.

Most people and even some surgeons don’t talk about this risk openly. THE MOMENT THAT CHANGED EVERYTHING

Back in 1997 or 1998, Dr. P was first introduced to the world of regenerative medicine — and it was a turning point that changed the course of his career forever.

At the time, he was completing his Sports Medicine fellowship under the legendary Dr. James Andrews in Birmingham, Alabama. One day, Dr. James Bradley, the team physician for the Pittsburgh Steelers, came to give a presentation. Everyone expected him to talk about the incredible surgeries he performed on professional athletes.

Instead, Dr. Bradley walked up to the podium and said something none of them expected:
“Our body has an amazing ability to heal itself.”

That statement caught everyone off guard. What did he mean by that?

He explained that he had been taking patients’ own blood, concentrating the platelets — the cells that help form a scab when you cut yourself — and using that platelet-rich plasma (PRP) to treat injuries. These platelets contain growth factors that stimulate new blood vessels and cells to come to the injured area, accelerating the healing process.

Dr. Bradley had used PRP on several Steelers players, including Hines Ward, who tore his MCL. Thanks to PRP, Hines Ward was able to return and play in the Super Bowl just two weeks later!

That single moment, Dr. P says, set the orthopedic world on fire — sparking an explosion into what we now call orthobiologics and regenerative medicine.

TO ICE OR NOT TO ICE???

You Just Had an INJECTION … Should You Ice or Not?
(Always ask your own doctor first)

It depends on what you had done—and this could make or break your results.
Most people think, “I should ice to reduce pain and swelling,” but that’s not always the right move. In fact, it could interfere with healing.

Here are Dr. P’s Three Secrets to knowing when to ice and when to walk away from the freezer:

1. Ice After Cortisone Injections, Hyaluronic Acid (H/A “Gel”) Injections, or Allergy Shots
These don’t rely on your body’s healing response—they’re often used to reduce inflammation in the first place (like cortisone and H/A).
If you flare up afterward, ice helps calm the joint down.

As for allergy shots (like the bee venom shots I get), the venom is injected just under the skin and absorbed slowly—not all at once into the bloodstream. Icing helps slow the absorption and decrease pain, local swelling, and inflammation.

You’re not blunting healing—you’re simply minimizing a side effect.

2. DO NOT Ice After PRP or BMAC Injections
These biologic treatments work because they trigger a controlled inflammatory response.
That’s what starts the repair process!

Ice constricts blood vessels and shuts down the very signaling pathways we want to activate.
Ice = Less Healing. Don’t do it.

3. Wait at Least 5 Hours to Ice After Laser Therapy
Our lasers—like the MLS M7 or M-Hi—dramatically increase circulation, oxygenation, and cellular activity.

Icing immediately after would cancel out those benefits.
Let the laser do its thing first. Then—maybe—ice after 5-7 hours if needed.

DO ALL POSTERIOR HORN MENISCUS TEARS NEED SURGERY?

Not necessarily. Dr. P breaks it down:

Complete radial tear? Yes, surgery is usually needed.
Frayed tearing but not unstable? No surgery needed.
Root tear? Almost always yes.
Horizontal tear? Sometimes yes, sometimes no. It depends.

The truth? It all depends on where the tear is and what type of tear it is.
There’s no cut-and-dry answer—and we never make decisions based on imaging alone.
