Just had a discussion with a patient and her husband about the outdated protocol R.I.C.E.

Here's some information regarding R.I.C.E.

The acronym R.I.C.E. (Rest, Ice, Compression, Elevation) was introduced in 1978 by Dr. Gabe Mirkin as the go-to method for acute injury care. For decades, it became standard advice for sprains, strains, and soft tissue injuries.

In March of 2014 Dr. Marking refuted R.I.C.E.

Why R.I.C.E. is Now Considered Outdated

1. Rest – Prolonged immobilization slows healing. Current evidence favors active recovery and gentle movement to stimulate blood flow and tissue repair.

2. Ice – While it can temporarily reduce pain and swelling, ice also constricts blood vessels, potentially delaying inflammation and the natural healing process. Inflammation is part of how tissues repair themselves.

3. Compression & Elevation – These can still help with swelling, but they don’t address the need for movement and circulation that modern protocols emphasize.

The Problem with Outdated Practice

Many physicians, trainers, and even some physical therapists still default to R.I.C.E. because it was taught for decades.

This reflects a knowledge gap: research has moved forward, but practice often lags behind.

Patients may receive outdated advice that doesn’t promote optimal recovery, potentially slowing healing or encouraging chronic issues.

In short, R.I.C.E. is still helpful for immediate pain relief, but it’s no longer considered the best standalone approach for healing. The shift is toward movement-based and education-driven recovery protocols.

Help Yourself Instead of Paying Another

As health care professionals, our goal should not be “how many times can I treat this patient?” but rather “what can I teach them so they can help themselves and reduce the need for care?”

One of the most common non-spinal problems I see in my office is forearm, elbow, and wrist pain—sometimes with hand weakness. Many patients have been told they have carpal tunnel syndrome.

In most cases, the real cause is adhesions and tightness in the forearm muscles. This creates tension either where the muscles attach near the elbow or at the wrist.

Why pay someone to treat this problem when, at no extra charge, you can be taught to self-treat using my official release, stripping, and stretching techniques? These methods release muscle tension and reduce symptoms.

Another condition you can learn to treat at home is plantar fasciitis. And there are many more you can manage yourself instead of paying for ongoing treatments. Of course, some conditions do require professional care—but many do not.

Ask yourself: Are you paying for something you could do yourself?

Remember—many myofascial syndromes result from repetitive activity. That means the problem will keep coming back unless you learn how to address it yourself.

You can keep paying for treatments… or you can learn how to fix it.
