Long Island Elder Care LCSW, PLLC was founded in April, 2006 with a mission to provide homebound counseling to the elderly and disabled. The company has since expanded our program to include Case Management to assist our clients and their families to set up services needed to maintain them in the community. Long Island Elder Care is committed to working with out of state families to set up services for their parents locally. Consultations are arranged to evaluate for Community and Chronic Care Medicaid. Consultations for Elder Care Attorneys are arranged to evaluate and implement services at home. We have Licensed Certified Social Workers that work for Long Island Elder Care to provide compassionate quality care to our clients
The Positive Connection is a subsidiary to Long Island Elder Care providing marital, group, and individual counseling at our 732 Smithtown Bypass (Suite A*5) Smithtown location. We are starting group sessions during the summer 2008 for children ages 11-18 to develop social skills in our 8 week program. We also have Anxiety and Depression groups starting in the Fall of 2008. This will be a 12 week educational and therapeutic program. SERVICES

HOMEBOUND COUNSELING
We provide homebound counseling to senior citizens and the disabled who would otherwise not be able to travel to an office for services. These services are provided by a licensed certified social worker. We cover most of Suffolk county. THESE SERVICES ARE COVERED BY MEDICARE AND A SUPPLEMENT, AS WELL AS MOST PRIVATE INSURANCE PLANS AT LITTLE OR NO COST TO YOU. CASE MANAGEMENT SERVICES
Our case management services provides comfort and relief to relatives who are out of state and cannot personally care for the loved ones. We do an initial assessment to determine what is needed to maintain your loved one at home. We provide supervision to ensure the safety and comfort of your relatives in their home. We maintain contact with the out of state relatives to ensure continuity of services. In addition we asses for entitlement and community services upon our initial visit. These services are payed for privately by the client and or family. The frequency of case management services are determined by the mutual consent of all parties involved. Consultations
We provide consultations for elder care planning in the privacy of your loved ones home. Assessments can be done in Nassau and Suffolk Counties at a mutually convenient time. We assess for cognitive functioning, safety,emotional status and whether your loved one is eligible for Community services and Entitlements. We have over 20 years of experience in working with the elderly and disabled. We are knowledgeable about Community Resources and are experts in Community and Chronic Care Medicaid. We can help with an interim plan while waiting for entitlements. This provides reassurance that is invaluable for families and gives direction for Elder Care planning. Paula Pagan , LCSW-R has over 20 years of experience in the field of Social Work. She had been successfully providing Psychotherapy to individuals (of all ages) and groups. She is available to provide you with the supervision needed to obtain your "R" number. We can set up supervision either in our Smithtown Suite and or in a location mutually convenient. We apply on behalf of the client for Medicaid in Suffolk and Nassau county as well as New York City. We apply for both community and chronic care Medicaid. The evaluation for these services can be done in the privacy of your home or our Smithtown location. Community Medicaid: Many people are eligible for community Medicaid but are unaware of how to apply for these services. If your loved one has community Medicaid they may be eligible for personal care aide services, adult day care programs, long term health care services, as well as an array of other services. We provide an initial assessment to determine eligibility and discuss treatment options. If you decide to utilize Long Island Elder Care to apply for community Medicaid services we would be responsible for the face to face interview with Medicaid and we would set up all the services upon the approval of Medicaid Chronic Care Medicaid: We apply for Chronic Care Medicaid for those loved ones who need to be place in a nursing home for long term care. We are experts in the rules and regulations governing the Medicaid program and we provide a seamless transition from community care to the nursing home. We can also assist you in the process of applying to various nursing homes and expedite the application process. We have Registered Nurses who will complete the patient review instrument and screen which is required for nursing home placement. SOCIAL WORKERS:
