We’re so proud of the progress Ms. M made during her time with us at Evergreen! She joined our AHA Heart Failure program with a focus on building her endurance and managing her heart health. ❤️

Through consistent therapy and specialized care, she maintained excellent stability and reached her goal of an independent discharge. We are thrilled to see her back home with her family, feeling prepared and confident in her daily routine. Way to go, Ms. M!
