It’s MBTUESDAY!
I got 2 Tips For You!…but first…
Did you know….On average, about 70-95% of maxillary first molars have an MB2 canal, but it may be difficult to locate without enhanced visualization tools like a dental microscope.

2 Tips for Locating and Instrumenting the MB2 Canal in Maxillary First Molars

1. Preoperative Assessment
• Use CBCT Imaging: This helps visualize the MB2 canal’s presence, trajectory, and curvature before starting.
• Analyze the Pulp Chamber Floor: A darker groove running from MB1 to the palatal canal often indicates MB2.

2. Proper Access Cavity Design
• Extend the Access Slightly Mesially: The traditional triangular access should be modified into a rhomboidal or slightly oval shape to expose MB2.
• Remove the Dentin Shelf: The MB2 canal is often hidden beneath a dentinal overhang—gently troughing with high speed round bur or ultrasonics.

The SHORT Case Of The Day!
Tooth #19 ~ Previous Trx With Metal Thermafil

Patient came in with pain with a previous root canal performed almost 20 years ago. Radiographically the root canal appears to be fine however pathology was present on the mesial and distal. A LFOV 3D CBCT was taken no crack was noted.

A retreatment was performed on this case. Upon access, there were metal Thermafil carriers. These are very difficult to remove. Be careful to expose the top of the carrier and not drill them down. I then like to use Chloroform to soften up the cement. Then use small microforceps to grab the metal carriers. Be careful trying to use ultrasonics. It can cause the carriers to break off deeper inside the canal making it harder to retrieve. Wire loop, EndoCowboy, and other devices can be used as well.

The case was obturated with single cone condensation with bioceramic sealer. Patient reported to be asymptomatic a few days later. They will return for a 6 month recall to evaluate osseous healing.

The SHORT Case Of The Day!
Tooth #30 ~ "I Don't Want My Tooth Pulled"
A 17 year old was in pain for over 1 month on tooth 30. The mom took her to a local dentist. The dentist advised her to have the tooth extracted because they did not do root canals. The mom decided to take her to Columbia South America to get the dental work done. When she got to Columbia the dentist said they didn't do root canals just extractions. The mom was very disappointed. She then heard about our office here in Atlanta.

She came to see me for a pain-free root canal experience in under 1 hour. She said I wish I knew you did this so well prior to my trip to Columbia. She was very grateful we were able to get her daughter out of pain and save her tooth.

The SHORT Case Of The Day!
New File Alert ~ ZenFlex CM by Kerr Dental

As an independent clinical consultant and KOL I get to try new files before they hit the market from several companies. Today I had the pleasure of using the ZenFlex CM (CM -Controlled Memory) file system by Kerr Dental.

Endodontic treatment was performed on tooth #15. I used Kerr Elements cordless rotary handpiece and Apex Connect (apex locator) for the working length. This handpiece is very light and can be preset to just about any rotary file. ZenFlex CM files were used to clean and shape the canals. They were very efficient, flexible, and strong. ZenSeal Bioceramic Sealer was used in a single cone technique. It's a very smooth bioceramic sealer with great radiopacity.

This case came out amazing. I was very impressed with the new improvements Kerr Dental has made in their endodontic line of products.

The SHORT Case Of The Day!
Tooth #14 ~ Anatomy & MSEO

Patient came in with pain on the upper left side. A LFOV 3D CBCT revealed pathology on teeth #14 and #15. Endodontic treatment was performed on both teeth in 2 separate visits. The diagnosis was a necrotic pulp with acute apical periodontitis with MSEO (Maxillary Sinusitis Of Endodontic Origin).

Both cases were obturated with ZenSeal bioceramic sealer by Kerr Dental. Notice the lateral canals picked up due to the nice flow-ability of this sealer on tooth 15. There was a separated file in the MB2 on tooth 14 due to the severe curvature. I did not want to remove it and risk a perforation. I left it and told the patient. Studies by Spili and Crump shows there is no change in the success rate. Also this happened towards the endo of cleaning and shaping.

If you have more questions about this case please comment below. Also I discuss a lot more detail in my textbook "Endodontic Keys And Cases" on Amazon.
