✅ Key Responsibilities
* Conduct **initial and ongoing client assessments**
* Develop, review, and update **individualized care plans**
* Complete **90-day reassessments and follow-up evaluations**
* Provide **client and staff orientation** and clinical guidance
* Coordinate with management to **schedule staff for client shifts**
* Ensure compliance with **NC DHHS and agency policies**
* Communicate effectively with clients, families, and caregivers
* Maintain accurate and timely clinical documentation

🎓 Qualifications
* Current **North Carolina Registered Nurse (RN) license** (required)
* Minimum **1 year of nursing experience** (home care preferred)
* Strong assessment, care planning, and organizational skills
* Ability to work independently and manage multiple tasks
* Knowledge of home care regulations and best practices
* Reliable transportation and valid driver’s license
* Professional, compassionate, and team-oriented demeanor
