Dr. Shahla Kaukab is a licensed physician operating in Soddy Daisy, TN. She is Board Certified and has been practicing for over nine years in the Soddy. Dr. Kaukab Naseer is a licensed physician practicing in Red Bank. He has been a Board Certified member of the Comprehensive Medical Group for 18 years in the greater Chattanooga area. We accept all insurances however we do not accept out of state Medicare. ALL Patients must bring:

(this is required of all NEW and OLD PATIENTS unless previously provided )
Physicals: Due every 12 months after the age of 2.*

*1m, 2m, 4m, 6m, 9m, 12m, 15m, 18m, and 24 months (2 year) physicals are required for all newborns to get properly vaccinated. Physicals are then scheduled every 12 months after 2 years of age. ADHD/Behavioral Consults: Due Every 3 months**. Bring teachers' notes with each visit.

Asthma: Every 3 Months

New Patients: must be scheduled before 3:30 p.m. Sports Physicals: Due 30 days before the start of sport***

***separate physicals done for each sport season (Football, Basketball, Baseball, etc.) Dental Pre-Op: Due Within 30 days of surgical procedure.

All our milestones are from American Academy of Pediatrics (AAP) and Center for Disease Control (CDC) materials. If your baby is missing a milestone, discuss this with Baby's healthcare provider.
