Did you know that “preventive visits” and “office visits” serve different purposes so they have to be billed separately?

Here’s a summary of how it works...

1. Preventive visit purpose: annual physical, preventing illness, catching issues early, and setting baselines. Preventative visits may include: screenings (blood pressure, glucose, cholesterol), vaccine review, counseling, STI/colorectal screenings as appropriate, and an overall risk evaluation.

2. Office visit only purpose: to manage a known condition or a new concern. Examples include: new or worsening symptoms, medication changes, test-result review, referrals, follow-up after a hospital stay, or chronic condition check-ins.

--> We want to be up front to make sure our patients understand that if a specific problem is addressed during your preventive visit, then we must bill for an additional office visit for that evaluation. We’ll do our best to be transparent about this at check-in.

To learn more about this and see more useful examples, visit our website: www.primarycaresb.com/visit-types

Five years ago, we opened our doors with a simple but powerful mission: to provide excellent medical care in a setting that respects the dignity of every person. We envisioned a practice that feels like primary care should feel like. Where the phone is answered by someone who knows your name, your messages get a thoughtful reply, and you have enough time with your provider to truly be heard.

In these first five years, we’ve...

1. Grown from a small independent office into a thriving community practice, even through a global pandemic.

2. Used technology (including AI) to make care more personal, not less.

3. Expanded our services into diabetic education, ultrasound, X-ray, mental health counseling, Saturday hours, sports physicals, community clinics, Botox, and acupuncture.

4. Welcomed new team members who share our heart for compassionate care.

Every person who walks through our doors is part of this story. Thank you for trusting us with your health and for helping us build a practice where care still feels human. We appreciate you.

Here’s to the next five years! 🫶

Halloween is all about fun, costumes, candy, and family time. Here are a few quick reminders to keep your night safe:

🎃 Check candy wrappers before letting kids dig in.
👻 Make sure costumes fit well and don’t block vision.
🚦 If trick-or-treating, use reflective gear and bring a flashlight.
🦷 Balance the sweet treats with water and teeth brushing!

If you have any concerns or want to schedule a check-up after all the fun, call us at 574-251-1200 or visit www.primarycaresb.com.

Running low on your medication? We’re here to help you stay on track.

- Fastest option: request a refill through the patient portal.
- Dose change or new medication? Please call to schedule an appointment so we can review your needs safely.
- Request refills before you run out so there’s no gap in treatment.

Questions or need to schedule? Visit www.primarycaresb.com or call 574-251-1200.

Important Note: Your provider needs to see you regularly to understand if the medication is still effective. Make sure you schedule your routine follow-ups and Annual Physicals. If it has been a while since you have been in, or missed appointments, this could impact our ability to send refills.

“Dr. Zabukovic is a breath of fresh air! He has a terrific bedside manner and takes ample time to get a handle on the patient’s situation. One walks away with a feeling that he CARES deeply for his patient. We couldn’t ask for more from a doctor.”

If you’ve ever been cared for by Dr. Z, you know what makes him special. Patients often say he goes beyond medicine — he takes the time to listen, remembers details about your life, and makes you feel seen. It’s no wonder so many in our community call him “my doctor” with pride.

At Primary Care Partners of South Bend, patients choose Dr. Zabukovic for thorough, compassionate care. He listens, explains options, and partners with you on next steps.

Sometimes what looks like a simple skin infection can turn painful fast. The good news? You don’t need to rush to urgent care or the ER. At Primary Care Partners, we’re able to treat many abscesses (infections under the skin) right here in-house.

👉 If you notice swelling, redness, fever or pain that doesn’t improve, call us to schedule a visit. We’ll evaluate and provide the right care on the spot.

Please don’t squeeze or try to drain at home. Our providers can evaluate and, when appropriate, perform an in-office incision & drainage and prescribe treatment to help you heal quickly. Schedule at www.primarycaresb.com or call 574-251-1200. Established patients can also message us in the patient portal.

As we prepare to head indoors this fall, it’s important to be aware of RSV (Respiratory Syncytial Virus). RSV is a common virus that can be serious for infants, older adults, and people with weakened immune systems.

RSV is the leading cause of hospitalization for kids under 5. It spreads easily through coughing, sneezing, close contact, and even touching surfaces.

-- For Infants: A one-time injection (Beyfortus) helps protect babies under 8 months, and certain toddlers up to 19 months.

-- For Adults: The Abrysvo vaccine is available for adults 75+, adults 60–74 with chronic conditions, and pregnant women (who may also pass protection to their newborn).

If you have questions about RSV or want to know if these preventive options are right for your family, our team is here to help. Call 574-251-1200 or send us a message on the patient portal.

Today, September 11th, we pause to honor the 2,977 lives lost on that tragic day in 2001. We remain humbled by the courage of selfless first responders who ran toward danger to save others, 441 of whom made the ultimate sacrifice.

In their memory, Dr. Zubukovic, Dr. Griffin, and Nurse Shelly joined the ND110 Stair Climb charity event last night. They climbed 110 flights of stairs, meant to symbolize the 110 stories of the World Trade Center, as a tribute to the extraordinary strength of the firefighters, police officers, and EMTs who served on 9/11.
