Hello Families and Guardians,

It’s that time of year again, for the Portland Wheelers to visit.

If your loved one is interested in the bicycle rides they provide, please see our Activity Director Jennifer, and she can supply you with a consent form.

We will need these forms filled out for ride participants and participation.

Jennifer has placed a stack of blank forms in the door entry way, you can pass these on the the nurses stations after they have been filled out.
