Qualifications:
• Education- Must have a high school diploma, or a GED, or must demonstrate the ability to read written personal care services assignment and sufficiently write required forms and reports of visits to clients.
• Training- Must successfully complete and pass the 40- hour personal care services curriculum training course and standardized examination designated by the Division of Medicaid. A satisfactory completion of a nurse aide training program for a hospital, nursing facility, home health agency, personal care agency, or
• Experience- Sufficient experience in managing a home or in care giving is preferred but not required. Demonstrate the ability to perform daily tasks with the understanding of what is involved. Personality attuned to the requirements of meeting needs of the infirm and aged. Personal care assistants must be 18 years old or older.
• Other requirements- First aid and CPR certification. Must have reliable transportation.
Duties and Responsibilities:
• Assist the client with activities of daily living such as bathing, personal hygiene, ambulation, transferring, and dressing.
• Ensure the safety and well- being of all clients at all times. In emergency situations, call ambulance or fire department.
• Assist clients as required and as directed by the manager according to clients services checklist provided to you.
• Complete daily reports of services provided
• Immediately report incidents to manager and complete incident report
• Receive messages and relay those to the manager in a timely manner. Refer questions or complaints to the manager.
• Keep manager updated on any changes in the status of the patient’s emotional, physical, and/or psychological state.
• Be instructed in continuing education annually.
• Other job duties as assigned by the Supervisor/Manager.
