Dear Southfield Obgyn Patient:

Southfield OB/Gyn Associates would like to advise you that Dr Patrice Harold and Dr Michele Thomas will be closing the practice. Dr Patrice Harold’s last day of providing medical care in office for patients will be effective May 22, 2025 and Dr Michele Thomas last day of providing medical care in office will be effective June 12, 2025. After 6/12/2025 for urgent matters you may go to an urgent care provider of your choice or in case of an emergency go to the nearest emergency room. You may reach out to the DMC provider referral service at: 888-362-2500 or the Henry Ford /Providence provider referral service at: 800-627-8106 to obtain a new OB/Gyn provider. Dr Thomas will be moving to a new office in the future, however details have not been confirmed at this time.

Once you have found a new provider, please complete the attached medical records release and send to:
29255 Northwestern Hwy. Ste 301, Southfield MI 48034 or fax to 248-354-2220.
All requests must be received by 07/15/2025. Please ensure all balances are paid before requesting medical records.

Starting Week of June first the office will be back to regular hours.

Effective immediately : The office will be open on Monday and Thursday from 845-1200 due to the Covid19 outbreak and the Governors executive order. We will only be seeing pregnant patients and emergent visits. If you have a matter that needs to be addressed please leave a message at the office or via patient portal to request a teleconference appointment. Please do not overwhelm our answering service with non urgent or prescription request , we will not be able to handle these request. Thank you and stay safe!
