✔️ This anonymous and confidential survey is part of the Community Health Needs Assessment (CHNA), which is conducted every three years to identify and prioritize health-related needs among county residents, set an action plan to address those needs, and improve the overall health of our community.

❓ How long will this survey stay open?
✔️ Brunswick County residents can take the survey from August 27th to October 3rd.

❓ Will my answers be kept private?
✔️ Yes; all responses will remain confidential.

❓ How long will it take me to finish the survey?
✔️ Estimated completion time for the survey is 15 minutes.
