Dear Patients and Families,
I would like to express my sincere gratitude and thanks for the trust you have placed in me and my staff at Yum Pediatrics over the past 10 years. Taking care of your child has been the greatest honor for us. Your child’s continued health and wellness is very important to us. I am writing today to inform you that I will be closing Yum Pediatrics this year on November 3rd.
This decision comes as a result of a number of factors, including some personal health challenges. I apologize that we were not able to give you more notice. After some rest, I intend to focus on family and health. I am also looking forward to putting my energy and 24 years of pediatrics experience into my public health passion through the nonprofit I founded, The Dr. Yum Project.
We will do our best in the next few weeks to see as many children for well visits, with preference given to those who need vaccines. We will open during limited hours by phone from November 5th- 30st to help transition remaining records and to conduct limited behavioral health telemedicine visits, and renew prescriptions for meds and therapy. Our wonderful office staff will be working closely together to ensure a smooth transition into your new medical home.I have emailed you a list of excellent providers who will be happy to see your child for continuing medical care.
I am mailing each family an authorization form for you to complete and return to our office to have your medical records forwarded to a new healthcare provider of your choice. Our office will be handling a high call volume, so please do not call the office for questions about records. Instead, once you are registered at a new practice, email the authorization back to office@yumpediatrics.com or upload it as an attachment into a portal message.
Once the office is permanently closed for all business on November 30st, your medical records will be placed in storage. Future medical record requests can be sent to office@yumpediatrics.com, and may be subject to a records fee. Requests will be filled within 30 days.
Thank you for trusting me with your healthcare needs throughout my time at Yum Pediatrics. It has been a pleasure caring for this cherished flock of children and families. I am also so grateful to the strong, supportive women pictured here. Our practice would not have thrived without them and I will miss working with this team every day.
