Good afternoon!
We hope everyone is doing well.
June is coming soon and it is Crescent Community Clinic sweet 16th birthday month!
For 16 years Crescent Community Clinic has been dedicated to providing essential healthcare services to those in need in our community. To mark this milestone, we're inviting you to be a part of this celebration by making a $16.00 donation!
Your $16.00 donation will make a meaningful difference in the lives of individuals and families who rely on our clinic for medical and mental health services. Crescent Community Clinic provides these services to adults between the ages of 18-64, who reside in Hernando County, and who do not have access to health insurance. Whether its covering the cost of a patient's medication through the Pharmaceutical Prescription Assistance Program, diabetes kits and monitors or even some medical supplies, every dollar counts and helps us continue our mission of serving those in need.

Together, we can ensure that Crescent Community Clinic continues to be a beacon of hope and healing for many more years to come. Let's make our 16th birthday celebration one to remember by spreading kindness and generosity within our community!

To donate, you can go on our website at www.crescentclinic.org where you are able to use PayPal, debit, credit card, or check in honor of our 16th birthday. Your support means the world to us, and we are grateful for your continued partnership in making a positive impact.

Thank you for being part of our journey and for helping us make healthcare accessible to all.

With heartfelt appreciation,

Christina Reyes

Crescent Community Clinic is seeking a retired nurse who can be of assistance to our PPA coordinator. The Pharmaceutical Prescription Assistance Program provides expensive medications free of charge to uninsured patients who have been denied Medicaid and who are unable to afford medications, specifically insulin, among others. The PPA coordinator then fills out an application and the pharmaceutical provides the specific medication. The PPA must be renewed yearly. If anyone has an interest in volunteering with Crescent Community Clinic for this position, please do not hesitate to contact Christina Reyes, Executive Director at 352-573-7886. I look forward to hearing from you.

Our Story

The Crescent Community Clinic was founded to improve the health and well-being of Hernando County adults 18-64 who are under served, indigent, and uninsured, to promote health literacy and to establish alliances that serve our community, and to provide the best health care regardless of ability to pay.

Patients are seen by appointment only. The Clinic does not provide emergent care (if you have an infectious disease or an injury we will refer you to urgent care or an emergency department). Our volunteer providers treat chronic medical illnesses and mental health concerns. To apply for services come to the Clinic during business hours and complete an application and be scheduled for an eligibility interview. The Clinic is open 9:00 am to 3:00 pm on Monday, Tuesday, Wednesday, Friday, and Saturday. The office is closed on Thursdays and Sundays. We are located at 5244 Commercial Way, Spring Hill, in the Wi******er Plaza (across the street from the big gray dinosaur).
