Position: Home Health Intake Coordinator
Pay Rate: $17 - $25/hour (based on experience)

Job Description:
ProCare Home Health is seeking a skilled and detail-oriented Intake Coordinator to join our dedicated team. This position plays a critical role in ensuring seamless onboarding of new patients and effective coordination of care.

Requirements:
• Experience: Previous experience in home health care is required.
• Software Knowledge: Experience with Kinnser (WellSky) and CarePort is highly preferred.
• Skills: Strong organizational and communication skills, with the ability to handle a fast-paced environment.

Responsibilities:
• Process and manage referrals for home health services.
• Coordinate patient intake and ensure accurate documentation.
• Communicate effectively with patients, families, and healthcare providers.
• Utilize Kinnser and CarePort systems to manage patient data and workflows.
