In Honor of National Midwifery Week

Dear Mama Afua,

In celebration of National Midwifery Week, I want to take a moment to honor you and express my deepest gratitude for the wisdom, patience, and heart you’ve poured into this sacred work.

Learning under your guidance was a transformative experience. You’ve shown me what it truly means to serve families with compassion, respect, and reverence for the birthing process. Your example has taught me that midwifery is not only a profession—it’s a calling rooted in love, strength, and the courage to protect physiological birth.

Your mentorship has shaped me in ways that extend beyond clinical skills. You’ve modeled a seriously strong presence, intuition, advocacy, and the art of trusting the body’s divine design. I am deeply thankful for every lesson, every birth shared, and every moment you reminded me that being a midwife is both science and spirit.

Thank you for being a light in this work—for standing firm in your calling and inspiring younger midwives like myself to do the same. May this week remind us of how deeply you are valued and how many lives you’ve touched through your care.

Shoulder Dystocia -

Shoulder dystocia is an obstetric emergency that occurs when, after the baby’s head is born, one or both shoulders become stuck behind the mother’s pelvic bones, preventing completion of the birth without additional maneuvers.

It is most commonly caused by the anterior shoulder becoming impacted behind the p***c symphysis. A hallmark sign is the “turtle sign”—when the fetal head retracts slightly against the perineum, resembling a turtle pulling back into its shell.

Key Points:

• Prompt recognition and intervention are critical to prevent complications such as brachial plexus injury, fetal hypoxia, or maternal trauma.
• Management involves a sequence of maneuvers (e.g., McRoberts position, suprap***c pressure, or delivery of the posterior arm) to relieve the obstruction.
• Risk factors include fetal macrosomia, gestational diabetes, previous shoulder dystocia, and prolonged second stage of labor, but it can also occur without any risk factors.

Midwives should be skilled in the identification and management of shoulder dystocia and prepared to act quickly with a calm, coordinated response.
