AJOG Expert Review in Labor: A critical evaluation of the external and internal maneuvers for resolution of shoulder dystocia - All-fours position and knee-chest position
A, All-fours position (knee-hands position). The mother is supported by her hands and knees. The hips are semiflexed. The spine is horizontal. B, Knee-chest position. The mother is supported by her forearms and knees. Hence, the hips are more flexed to simulate a reverse McRoberts maneuver. "Midwifery models of care are models of care in which the main care providers for women and newborns, starting from pre-pregnancy and continuing all the way through the postnatal period, are educated, licensed, regulated midwives who autonomously provide and coordinate respectful, high- quality care across their full scope of practice, using an approach that is aligned with the midwifery philosophy of care, which:

i. promotes a person-centred approach to care;
ii. values the woman–midwife relationship and partnership;
iii. optimizes physiological, biological, psychological, social and cultural processes; and iv. uses interventions only when indicated.
In midwifery models of care, midwives provide integrated care, addressing the needs of each individual woman and newborn, within functional and enabling health systems, equipped with necessary resources and streamlined consultation and referral processes. They collaborate within networks of care as part of interdisciplinary teams characterized by equality, trust and respect. This approach guarantees that every woman and newborn receives personalized care, tailored to their health needs.

Midwifery models of care are adaptable to all levels of care and contexts, including home-, community- and hospital-based settings; the public and private sectors and public–private partnerships; resource-constrained environments; and humanitarian and crisis settings. This ensures wide accessibility, equity and relevance across different cultural contexts for women, newborns, partners, families and communities."

I love these people so very much. TLC Doula Group is more than my co-workers, they are family. Overy 20 years working as a doula and I can’t imagine being anywhere else. The world has kept my feet on this path so that I can be in this moment. I wouldn’t change a thing.

Happy Lily Day, Baby! Twenty-four seems like such a big number. So long since we said hello and I held you like a brief and shining star. You burned yourself into my heart and left me changed in ways I never expected. I always want to say that you stole a part of me, but that's not really true, is it? I gave you a piece of me and you gave me a piece of you.... And when I feel the absence of you it is that part I feel, not you, but that piece of me that I gave to you. You live on inside me and your father. I see you every day in the way Kota laughs, in Gibson's delight in playing piano and in Lola's laser sharp focus. All these traits I know you would share with them. I get to see glimpses of you every day, in every moment. I miss you. It hurts how much I miss you, but the pain is essential. It means I had you. It means I remember.

Grief is not a moment. You were not a moment. I wake up every single day and you are gone. It never changes. And that keeps you with me. In my heart and on my mind.

I miss you. But you gave me who I am. Loving you makes me better. A better listener. A more forgiving person. A more thoughtful speaker. And I hope a soft spot for all people who struggle in this life. And I love you every single day - until there is nothing left of me but star dust.

While Kathleen specializes in high risk pregnancies, she loves to work in all environments with all different kinds of providers and families. Kathleen is also a co-founder of TLC Doula Group - Houston’s oldest doula collective and co-founder of Rebel Birth Education and Resource Haven, a support Haven for families in the child bearing years. Kathleen is a native Houstonian and has been married for over twenty years.
