Meet Susan Reed, PharmD, BCGP, Senior Consultant Pharmacist at Guardian Pharmacy of the Heartland. With nearly two decades of long-term care pharmacy experience, Susan brings deep clinical expertise and a passion for resident care to her role.
Since joining Guardian over five years ago, Susan has continued doing what she loves most: partnering with care teams to improve quality of life for residents. Three years ago, she stepped into a leadership role, guiding a team of consultant pharmacists known for their compassion, collaboration, and dedication to clinical excellence.
As a consultant pharmacist, Susan provides individualized medication reviews with a focus on resident safety, quality of life, and measurable outcomes. She partners directly with providers and community care teams to improve medication adherence, reduce hospitalizations, and minimize risks like falls, polypharmacy, and other preventable adverse drug events.
“I feel incredibly lucky to lead a team that is so supportive and caring,” Susan says. “We learn from each other every day, and that shared dedication makes a big difference in the care we provide.”
A graduate of St. Louis College of Pharmacy and a board-certified geriatric pharmacist, Susan is driven by the opportunity to help residents meet their health goals through medication optimization. Her commitment to clinical leadership and personalized care reflects Guardian’s mission to serve with compassion, integrity, and excellence across all long-term care communities.
