Rant time. I mean Story time! 😊

I had a 60 year old come in for a consult. She’s been following us on Facebook for years and is now needing help with her hormones. She went into menopause at age 52. She got along for a few years but finally begged her GYN for something to help with her symptoms. He gave her an estradiol patch 0.25%. Such a low dose though, all it’s doing is helping with hot flashes and night sweats. He also gave her “progesterone”. She was wanting more progesterone but said at her last visit, he lowered the dose. Thankfully she brought the bottle in and guess what the “progesterone” was. You guessed it. Provera!! Provera is the progestin that has a black box warning for breast cancer and the one in the WHI, women’s health initiative, that was stopped early because of obvious increased risk of breast cancer.

Of course I asked if she was given an informed consent about the black box warning, and like all the others, her answer is no. This is a “women’s expert”. A GYN! Remember, even those tasked with women’s health, like GYNs, DO NOT learn anything about menopause. No specialty in medicine learns about menopause. Shocking!! Especially because you don’t have to be super duper smart to know that 100% of women will experience menopause. We don’t know how many will get hypertension or fatty liver but we know EVERY SINGLE WOMAN, if she lives long enough, will go through menopause.

A couple things throughout her journey happened. When she first started hormones she forgot to take her “progesterone” for a couple days and had bleeding. She called the GYNs office and they put her through an endometrial biopsy!! As usual, without any pain relief. Women rarely get offered pain medicine because we’ve had babies, we’re tough. Whatever 🙄 If she was my patient and had bleeding, and had only been a few years out of menopause, I would first order a pelvic ultrasound. Then, if needed, referred for endometrial biopsy. It is NOT UNCOMMON for women to have 3-5 menstrual cycles within the first 5-7 years of menopause. I don’t freak out.

Another very, VERY sad part of this patient’s story is her and her husband went to a conference and listened to a physician, who I actually like. This female doctor told this room of people to never take hormone replacement. So this patient stopped her hormones. She said within 2 weeks she felt absolutely horrible and went back on them. I wonder how many women this doctor has prevented from giving life supporting care because of misinformation. This patient only knew how good she felt ON them, and restarted them. Other women won’t even try now. This patient watched her mother suffer from menopause and doesn’t want to repeat that.

What to do now? Go look at your bottle of “progesterone” and make sure that’s what it is. Not medroxyprogesterone or norethindrone. The mirena IUD, depo provera, the mini pill are also examples of progestins. As a nurse we used to give megace all the time for an appetite stimulant to patients in the hospital. I never knew that was also a progestin. I also had a GYN at Mercy tell me I needed birth control but “Don’t worry, we have a progesterone only birth control pill.” No, Sir. You don’t!! These are the providers some of you are asking for BHRT or advice on the BHRT you’re on. It’s like asking a neurosurgeon how to install your plumbing.

All I do in the office is hormone replacement. All day long. I was certified in hormone replacement through World Link Medical, in 2015. More training in BHRT than your GYN, as crazy as it sounds. I just wish when you went into them for BHRT they referred you all to people like me. Why can’t they just say, “I didn’t learn that”? If you knew how many times I say to patients, I’m not well versed in that. I don’t know anything or much about that. My ego isn’t too big that I am unable to say I just don’t know everything.

I put this patient on ACTUAL PROGESTERONE, oral estradiol, and testosterone. We will see how it goes. I hope this helps. ❤️❤️

I know my posts are long (trust me I write them 🤪) but there is detail and nuance that sometimes needs explaining.

What every woman should know about estradiol. Estradiol is the estrogen that women mostly make and the one responsible for most of the benefits. We also call it E2 based on the structure. You also make estriol E3 and estrone E1. The best way to get estradiol is through the oral route. When it goes through the liver it gets the first pass liver effect. This then has benefits for the heart and lipids. Patches or cream (transdermal) will still give you all the benefits except all the heart benefits. When a patient comes to me on E3 or E1 cream, I know a provider doesn’t understand hormones. I do use a E3 cream with hyaluronic acid, and vitamin C. But I don’t use E3 for any of the other benefits.

Most women can be offered oral estradiol. Only woman with clotting disorders or metabolic disorders will I give cream. Clotting disorders like Prothrombin and Factor V Leiden. Not because oral estradiol causes an increased risk of clots for heart attack and stroke, but because corporate medicine thinks it does and they will blame me for a heart attack or stroke. In the end, I’d be cleared after expert testimony regarding all the studies, but that would cost a lot of $$$ and stress in legal fees.

Estradiol helps with hot flashes, night sweat, vaginal dryness, painful s*x, reduction of cholesterol, prevention of osteoporosis, heart disease, and Alzheimer’s/dementia. A side note, men need estradiol too and they convert their testosterone to a bit of estradiol for the same benefits as women PLUS prostate protection.

At menopause, women are making less estradiol than a man. 🤯 Then women are gaslighted into believing this is “no big deal”. “It’s a rite of passage for a woman”. “You just have to deal with it for a short time”. “You’re getting old”. “It’s just a season that every woman deals with”. “It comes with age”. These are REAL STATEMENTS MADE TO ACTUAL WOMAN BY ACTUAL HEALTHCARE PROVIDERS!

I once had a 72 year old female who came to see me. She had hot flashes and night sweats ALL DAY, EVERY DAY since the age 52! Every. Day. I put her on all the hormones but within a week the hot flashes and night sweats went away. That was 20 Years! Can you even believe that people get talked to like this by a provider giving a service you’re paying for? I always say I’m nicer to the stranger in the grocery store.

So please don’t fear estradiol. Even if you’ve had estrogen receptive breast cancer, you can still go back on hormones, if you choose to and are comfortable. ❤️❤️

Mark and I were talking in the last few days about the wrath of God. How he sent down fire to kill people at times. 😳 I feel like, me included, have forgotten the wrath of God because we’ve been living in grace. I think if we talk about Grace of God, we should talk about wrath of God and I think if we talk about heaven, we should talk about hell. I love this verse because I always want to have wisdom or discernment. But it’s difficult to attain that if you don’t understand who God is. ❤️

What an amazing female hormone progesterone is. Why would anyone want to live without it? Please, no one expect me to live without mine.

Progesterone is the hormone of gestation. This is a big reason there are miscarriages. Anyone who has PCOS, history of a miscarriage, or an older woman needs to automatically be given progesterone throughout the entire pregnancy. Not just the first trimester. I was 36 when I delivered. This is considered “advanced maternal age”. I was out on bedrest at 18 weeks and delivered at 29 weeks, 1 day. You know what should’ve been prescribed?Progesterone.

Postpartum depression is progesterone. What do women get offered though? Anti-depressants, which have been shown in studies to be no better than placebo in depression but also NO HELP with hormonal depression.

Progesterone is the hormone of contentment, helps with anxiety, depression, sleep, evenness of mood, helps with libido and bit, helps with hot flashes and night sweats in perimenopause, heart palpitations, breast, brain, and bone protection, decreases PMS, and helps fibrocystic or dense breast tissue. This starts to decrease in your mid thirties. You know how you start becoming a “hot mess”? Saying things you’d never normally say? Getting fed up quicker than normal? Not sleeping? Becoming moody, irritable, and more rage feeling? Knowing that people are starting to avoid you because you’re acting crazy? THAT’S PROGESTERONE!

There’s a “catch” however. It is standard of care (which doesn’t mean standard of health) NOT to give progesterone to women who DON’T have a uterus. The corporate medical establishment believes progesterone’s only job is to down regulate estrogen’s effects on the uterus. And it does do that too! But did you read the paragraph with all the other benefits? This is a big pushback my patients get if they do end up telling their GYN about hormones, which I don’t recommend since they’re not experts on hormones. They berate them about not needing progesterone since they don’t have a uterus. To which a few of my patients have argued back that it’s the progesterone that is helping the most. Especially because these women still have their ovaries so they aren’t even on estrogen yet. 🙄😩🤦‍♀️

Progesterone and testosterone are the first hormones to go. However, women have already been living with low libido so they don’t notice the testosterone like they do the progesterone. I say the loss of progesterone smacks women upside the head.

The cost of progesterone at the compounding pharmacy I use is $101.04 for a 3 month supply of 200mg. 300mg is about $15 more every 3 months. I use MedQuest pharmacy in Utah

If you’re a provider reading this listen up👂. Stop withholding life preserving, life changing, and beneficial treatment to women who are suffering! JUST STOP! Either refer them out or go to conferences and get certified to help. Otherwise 🤐

We had a Wednesday sermon on this verse and it was so encouraging Our pastor said by submitting to God, you’re actively resisting the enemy. So simple. ❤️❤️

A late post from our meal on Sunday. We had company for dinner so I made some desserts. We had our usual steak, eggs, bacon, sautéed onions, and cauliflower mash. I usually make a veg on Sundays. I used Rosette's Mix sugar cookie mix as a crust, baked it. I made sugar free vanilla pudding and put it in the crust and added blueberry and raspberry. I also made Rosette’s peanut butter cookie mix. It was all gone. All the kids loves it too, even a 7 year old. There was Nothing left.

We don’t do dessert all the time but it’s nice to know there’s low carb options when you want it. It’s a great way to stay on track. ❤️

Providers act like morons when they consistently tell women “You’re too young to have hormone problems” and leave them to suffer. Hormone issues can happen at any age. Just look at PCOS (polycystic ovarian syndrome) and PMDD (pre-menstrual dysphoric disorder). These can become apparent as soon as menstruation begins. But we tell women you’re too young? It’s in your head? It can’t be that bad? Shame on you!!

In October 2020, I saw a 21 year old female, who was the daughter of my patient. She described issues since junior year of high school and they became worse in college. Her emotions were extreme with her menstrual cycle. She complained of irritability, sadness, brain fog, and when she looks at assignments and her computer she just can’t get started. She was Diagnosed with PMDD in high school. So the she was treated with the ONLY TREATMENT METHOD available and given……..you guessed it!! Anti-anxiety and anti-depression medicines. Of course! What else could it be! She didn’t take these for long.

Well I put her on compounded progesterone 200mg and she came in for her first follow up in March 2021. She said she is “definitely better”. Her Anxiety, brain fog are better, feels more calm, less anxiety, more even sleep. All better. Eventually, me**es got a bit shorter but less heavy. Sometimes progesterone will alter me**es and make them more frequent in the beginning and then even out again. She Feels a difference in her moods. Over time she moved up to 300mg and this dose is where she felt the best.

April 2022 got into OT (occupational therapy) school!! She said this would never happen before progesterone. She said overall doing great and in fact she has never been better. Me**es is better, not as extreme, moodiness better, eating better. January 2023 “feeling amazing”. Her mom is so grateful and says she has her daughter back!

January 2024 still doing great. Better than she’s ever been, mentally good and she is good, life is manageable. She went on to graduate OT school.

So if her mom wasn’t my patient, what would’ve become of her? Her life? Her wellbeing? Her future?

Providers, listen up!! Stop gaslighting women and saying there is nothing wrong!! Stop it!! Please, please do your job and help women through these types of issues. If you don’t know how to help, refer them out to someone like me that does hormones. Again, providers listen up!! Anti-anxiety and anti-depressant medications do NOT help hormonal issues. First of all, the studies on antidepressants are not very good for helping the issues they’ve been patented for. They’ve only been studied a year. The neurotransmitter theory has been debunked. WHY would we assume they have an anti-depressant deficiency when it’s clearly hormonal? Providers, again, listen up! Your first sign is when women tell you this happens, cyclically, monthly, and is associated with their menstrual cycle. You HAVE to do better!!! Hormones matter!! It’s all women are made up of. Do your job, please.
