Thyroid issues are often an overlooked root cause of chronic fatigue, and unfortunately, conventional medicine might miss it. Typically, when doctors test thyroid function, they primarily focus on TSH and sometimes T4 levels. If these are out of range, they’ll diagnose the thyroid as underactive or overactives. However, functional medicine takes a more comprehensive approach by looking at a 10-part thyroid panel, which includes testing for thyroid antibodies (such as thyroid peroxidase antibodies, indicating Hashimoto's), and checking for issues in converting T4 to T3—the active form of thyroid hormone that actually gives you energy.

Conventional medicine assumes the body converts T4 to T3 efficiently, which isn’t always the case. This means many individuals could have low T3 levels despite having “normal” TSH and T4 levels, leaving them feeling tired and sluggish. Furthermore, the range for TSH in conventional medicine is so wide (.4 to 4.0) that it often misses subtle imbalances that could be making you feel unwell.
