Job Summary:
As an RN at Trusting & Loving Care In-Home Services , you will be responsible for conducting weekly nurse visits, including monitoring vitals, setting up medications, and providing guidance and support to our entire care team. You will work closely with our clients and their representatives to assess their specific needs, develop individualized service plans, and coordinate care to ensure the highest level of quality and comfort.
Duties and Responsibilities:
• Training, evaluating, and supervising personal care aides to ensure excellence in care delivery.
• Assessing clients to establish their specific needs and developing comprehensive service plans.
• Planning, implementing, and coordinating care in collaboration with our care team.
• Conducting supervisory visits to evaluate care and provide ongoing support and guidance.
• Evaluating clients' homes for safety and accessibility issues and making recommendations for improvements.
• Communicating any changes to service plans promptly and effectively to the administrating team.
• Performing other duties as needed to meet the needs of Trusting & Loving Care In-Home Services LLC.
Qualifications:
• Valid RN license in the state of [State].
• Minimum of 2 years of nursing experience, preferably in home care or a similar setting.
• Strong clinical assessment and care coordination skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
