Medication routines can become complicated over time — especially for older adults or anyone living with chronic health conditions. When multiple doctors, new prescriptions, or health changes are involved, medication lists may no longer match what someone is actually taking. This can increase the risk of side effects, interactions, and even hospital visits.

We recently wrote a blog about the importance of doing regular medication reviews (sometimes called a medication reset or medication management) and how Care Management and In-Home Support can help keep medications organized, safe, and up to date.

As part of our private care management program, we provide medication review and medication set-up, including organizing weekly pillboxes and monitoring for changes, which can be especially helpful for individuals with complex health needs.
